This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.

The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.  

You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.

Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. Experience of working in mental health or dementia services would be an advantage.

About the role:

In October 2023, the organisation began delivery of Year 1 of their West Midlands based Environmental Leadership Programme in partnership with the Shropshire Wildlife Trust. The role of the Participant Support and Delivery Officer is to work closely with the programmes delivery team across the partnership on the different stages of programme recruitment, delivery and implementation – all to ensure that participants have an excellent experience with us.

Key areas of responsibility include:  

• Supporting the recruitment and onboarding of Year 2 participants in the run-up to the programme starting in October 2024 and provide a friendly point of introduction for participants onto the programme. This will include travelling to support the delivery of in-person taster and recruitment activities.
• Supporting the participants and managing their experience before, during and after programmes. This will include working with the programmes team to ensure we have collected all of the participant information we need to be able to assess the success and impact of our programmes and will include communicating with and keeping track of correspondence with participants
• Carrying out monitoring and evaluation activities and analysis of data that will help us measure and communicate our programme impact, including data collection (qualitative and quantitative) that can be used to create a programme narrative
• In Salesforce - undertaking data entry and administration including the production of basic reports and the ongoing development of simple automation that helps us streamline processes across the programme and partnership

About the Service 
Our Nightlight Service is proud to provide urgent support, crisis intervention and emotional support to people experiencing a mental health crisis in Hertfordshire as an alternative to statutory pathways.

We provide support to adults across Hertfordshire 7 days a week, 365 days a year through our helpline, crisis centres, crisis cafes and a weekend Crisis House. We aim to provide a safe space for people in crisis to talk and feel supported, be listened to without judgement, access relevant support with problem solving and facilitate access to external services when necessary.

The objectives of Hertfordshire Mind Network’s (HMN) Nightlight Mental Health Crisis Services are:

• To improve the mental wellbeing of people experiencing mental health crisis in Hertfordshire.
• To increase early access to help for people experiencing mental health crisis in Hertfordshire, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To provide an alternative pathway to people in distress and crisis to support better outcomes and reduce the demand on statutory crisis services and emergency services.

About the Role 
Mental Healthcare Support Workers are key members of staff delivering the Nightlight Mental Health Crisis Service, taking part in a rota covering 7 days per week, ensuring the delivery of quality crisis support services and instilling Hertfordshire Mind Network’s values throughout the crisis cafe and helpline provision. Working closely with other staff (bank staff, volunteers), coordinators, team leaders and Crisis manager to ensure services are of the highest quality and support the continued growth and development of the service.

Key Responsibilities

• Providing emotional support and developing, alongside clients, flexible and realistic crisis support/person-centred plans within agreed guidelines with support from leadership team (including assessments of need, risk, crisis support plans and review forms), often where risks are significant.
• Liaising with statutory agencies to ensure appropriate intervention is obtained for individuals.
• Drawing upon own lived experience as appropriate to inform the delivery of person-centred support.
• Providing immediate advice and information to clients to support with crisis presentations.
• To support individuals both through telephone support (helpline) and face to face support (crisis café).
• Liaising with Nightlight management and external services e.g. police, ambulance service, Crisis Resolution Home Treatment Team, Single Point of Access, Safeguarding Out of Hours Service, Mental Health Liaison Teams, Adult Community Mental Health Services etc .as needed in terms of accepting referrals and proactively recognising the indicators of deteriorating mental health and facilitating appropriate support.
• Completing appropriate signposting/onward referrals to both internal and external services in accordance with identified needs and risks.
• Supporting clients with making drinks and beverages as needed when visiting the crisis café.
• Assisting with housing keeping/cleaning tasks as necessary and reporting any maintenance problems.
• Contributing to the co-development of the service by supporting service user, family and carer involvement by obtaining feedback and supporting where necessary chances to be involved in wider service reviews.

• Annual leave entitlement of 25 days per year pro rata, rising in increments to a max. of 29 days after 5 years employment (plus 8 days Bank Holidays)

This role is an opportunity to work for Mind BLMK as part of the Milton Keynes NHS mental health service for someone with experience working in health, social care or information and advice, in direct contact with service users.

Working alongside colleagues from Central Northwest London Foundation Trust (CNWL) this role will provide referral management to and from Secondary Services. This will include signposting patients to statutory services including talking therapies and wider community based & voluntary sector support.

The Community Navigator will work as part of the team to deliver on the aims and objectives of the Mental Health Hub whilst representing the Voluntary and Community sector with the NHS.

Service Delivery

• Work within the NHS Mental Health SPA (Single Point of Access) Team to review and process referrals received from GP Practices and other sources including those to be subsequently triaged and assessed by mental health practitioners.
• Actively call patients with a view to assisting them through the process and making their onward appointments into community or secondary care service providers.
• Support the hub to signpost patients and direct them to appropriate services including secondary care, the local Voluntary and Community sector services and activities that support the patients’ wider determinates of their mental health and wellbeing.
• Answer calls, ensure sufficient phone cover, respond to messages/voicemails, provide administrative support to the Mental Health SPA for all services and deal with queries in a sensitive and timely way liaising with clinical or team lead as is appropriate.
• Monitor service inboxes during core office hours, responding to any queries professionally providing helpful information.
• Monitor and action tasks within SystmOne and maintain records of all activities carried out, i.e. outbound calls, calls received, archiving any received correspondence.
• Build good working relationships with the voluntary and community sector, relevant services and activities to ensure signposting pathways are kept up to date as well navigating and developing the setup of new signposting pathways through networking.
• Provide clinical and non-clinical information in appropriate chronological templates to other services.
• Carry out health and safety responsibilities as directed by Managers in line with Mind BLMK’s H&S policies, procedures, and guidance (risk assessments, reviews, reporting, safeguarding, incidents/accidents, premises).
• To bring VCSE experience, knowledge and culture into an NHS team.

You will work with young people between the age of 17-25 with needs related to substance misuse, including those that are affected by their parent’s substance misuse. Based in the Transitions team, you will support young people with multiple risk factors and substance use. Our Complex Needs Practitioners work flexibly across the community and within our multi-agency partnership settings. A core part of the role will be working with young people and their families to address holistic support needs across all aspects of the young person’s wellbeing. This could include diverse work related to homelessness, family work, exploitation, physical and emotional health as well as substance misuse.
 
An energetic and confident self-starter, you have proven experience within health and social care, particularly working with young people and supporting them through mental ill health.

This is a permanent full-time role requiring the post holder to work 37 hours per week.
