We are looking for new team members to join our service supporting Carers across Havering to improve their health and wellbeing, and access wider support that meets their needs.

This is a great opportunity to support local communities, working with Adult Carers and Young Adult Carers and helping them to make informed choices, build confidence, sustain their independence, and access support that meets their individual needs. You will carry out assessments to understand people’s individual circumstances and level of need, working together to co-produce support plans. This will involve meeting Carers in community settings and conducting home visits. The team also facilities coffee mornings and activities offering Carers opportunities for peer support and to take a break from caring.

You’ll build on your local knowledge to signpost and facilitate access for Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will support the development of escalation plans so that Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.

We have a part-time roles available (22.5 hours), and applicants should have relevant personal or professional experience of working with Carers and/or vulnerable adults in a relevant sector. It’s essential that you are confident using IT and maintaining excellent records. You’ll need a practical understanding of barriers faced by Carers, and an awareness of cultural differences and access to services.

Due to the area covered by the service, you will need a valid UK driving licence and access to your own vehicle.

Support Coordinator

We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join our Life After Stroke Service based across Cornwall.

This is an exciting opportunity to work with stroke survivors and their families to support them following a stroke.

Position: S11376 Stroke Association Support Coordinator

Location: Home-based, Mid Cornwall and West Cornwall. However, Frequent travel will be required as part of this role (to include team meetings or other work-related meetings)

Hours: Part-time, 24 hours per week x 2 posts available (one is a maternity cover contract)

Salary: Circa £19,400 per annum (FTE circa £28,300 per annum)

Contract: This is a fixed-term contract until 31 March 2027. Our services are contracted; we currently have funding for these contracts until 31st March 2027

The Role

The service aims to identify and provide key worker support to meet the needs of stroke survivors and carers across the stroke pathway. Providing a range of innovative support solutions, supporting them to meet their desired outcomes.

The Stroke Association Support Coordinator will:

• Support new stroke survivors and their carers from hospital discharge into the community.

RideWise is an East Midlands based charity helping people and communities travel more actively, sustainably and confidently. Through cycle training, led rides, community projects and practical support, we make cycling, walking and greener transport more accessible to people of all ages and backgrounds.

We are looking for a highly organised and proactive Operations and Resource Manager to play a key role in the smooth running of our cycle training and sustainable travel services. This is a hands-on coordination role at the heart of our charity, supporting our freelance instructor team, operational systems, project delivery and client relationships.

You will help make sure instructors have the information, resources and support they need, that records and compliance documents are accurate and up to date, and that our services continue to be delivered safely, professionally and effectively. This role is a part-time permanent contract designed to provide admin and practical support around donor-facing events and comms. Off The Record Bath & North East Somerset (OTR) is a mental health and wellbeing charity that gives local children and young people (CYP) a safe space to be heard and be themselves.  We provide a range of services for CYP, including counselling, listening support, youth participation, advocacy, support for care leavers and a LGBTQ+ focused youth group.

Purpose of the role

Our CAMHS and Hospital Youth Work Lead is an exciting new role, leading two highly impactful OTR services: the RUH Hospital Youth Workers and the BANES Child and Adolescent Mental Health Services (CAMHS) Social Prescribing Link Workers. You will lead the delivery and development of both services, including working closely with our partners at the RUH, in CAMHS and in the wider BANES community.

You will also:

· Manage referrals coming in to both the Hospital Youth Workers service and the CAMHS Link Workers service.

· Lead on monitoring the impact of both services and report regularly on this to our funders/partners.

· Engage in direct 1:1 casework

· Ensure strong safeguarding practice across the two teams.

The role combines team leadership, safeguarding oversight, partnership working and frontline youth work. You will help strengthen collaboration between hospital and CAMHS teams, community organisations and local services to ensure CYP are able access appropriate non-clinical support.

At the RUH you will oversee the service’s work with CYP aged 11–25 who present at the Emergency Department with low to moderate mental health needs, or who are admitted to hospital wards with long-term health conditions. This includes managing a limited 1:1 caseload alongside your project management duties. Utilising a person-centred and trauma-informed approach, you will build trusting relationships and support CYP to identify and access the support they need. Support may take place face to face or virtually and will focus on improving access to community-based support networks, with a view to improving wellbeing, increasing resilience and reducing repeat hospital attendance.

At CAMHS you will oversee the service’s work with CYP aged 10-17 who have been accepted into CAMHS with moderate mental health and wellbeing challenges. The Link Workers work with CYP at the start of their CAMHS support, alongside other CAMHS interventions, and those who are nearly ready to be discharged from CAMHS. Within this role, you will hold a small caseload of CYP, working with them for an average of 10-12 weeks. You will deliver social prescribing as the main intervention.

Working pattern and location

 

This role is primarily based at the Royal United Hospital in Bath, with some travel around B&NES to support the CYP on your CAMHS caseload. You would also spend some time each week at the CAMHS office in Keynsham. As an OTR employee, you would also have access to our office and meeting rooms in central Bath.

Occasional out of hours working may be required, for example to settle a young person into an early evening youth group or as a part of our safeguarding on-call rota.

About the Role

As a Sexual Violence Outreach Worker you will provide compassionate, flexible, and connected support to clients who have experienced childhood sexual abuse and/or adult sexual assault, in order to help them engage with the support provided by the new Integrated Plymouth Sexual Violence Service.

Many individuals accessing the service will have experienced multiple adverse experiences and may present with complex, intersecting needs such as insecure housing, substance misuse, domestic abuse, and significant mental health challenges. You will provide support that aims to meet people where they are in order to build rapport and trust as the foundation for longer-term support and interventions.

Key Responsibilities

• To build secure, consistent, and trusting relationships through a highly person centred approach as previous experiences and trauma can lead to mistrust of services, and inconsistent engagement.
• Support individuals to explore their holistic needs to increase their physical and psychological safety, sense of control and while working collaboratively with multi agency partners to ensure coordinated, wrap around support.
• Identify barriers to individuals accessing the service and determine solutions
• Actively promote and maintain a visible service presence within relevant community and partner settings to build trust and encourage referrals
• Utilise a case management system to maintain accurate and professional recording of case records and contribute to monitoring information for the service including the collection of outcomes

About the role

As Employment Service Manager, you’ll lead a small, dedicated team of Employment Specialists delivering high‑quality, recovery‑focused employment support across East Kent.
