About The Role

Salary: £22,125 - £23,472 per annum (FTE) pro rata'd for 21 hours.
Office Location: Community based in Warwickshire, with some homeworking elements.
Interview date: 24th April 2024.

About the role
We are offering a great opportunity to join our team providing personalised support to people diagnosed with dementia and their carers.
Our Dementia Connect service provides an opportunity to reach more people affected by Dementia through Telephone, Face-to-Face and Online support. The Warwickshire team are well-established, local experts in face-to-face Dementia Support with a range of experience and knowledge to tailor our community-based service to the diverse local population.
About you
If you have the qualities, we’re looking for and are passionate about improving the quality of life for people with dementia, then we want to hear from you in your Personal Statement!
We are looking to welcome an approachable, enthusiastic, and knowledgeable Community-based Dementia Adviser who will focus on supporting people affected by Dementia across the Warwickshire area.
Trusted Experts - you will be joining an experienced team of Dementia Advisers across the Warwickshire area, using a person-centred approach to provide expert local guidance, information and advice to people living with Dementia and their family/carers.
Better Together - we pride ourselves on supporting a diverse local population in Warwickshire, so an inclusive attitude and mindset is important to ensure we support everyone in the best way possible.
There is an emphasis on partnership working and networking with Health and Social Care professionals, including GPs, Social Prescribers, Memory Assessment Services and Social Services. You will need experience in networking effectively and collaborating with other professionals to achieve a positive outcome for service users. In addition, you will also have a focus on supporting “seldom heard Communities”.
Determined to Make a Difference – you will be passionate about ensuring that people with Dementia and their family/carers are able to live well, enjoy life and have their voices heard.
Compassionate – you will have a caring and empathic approach to supporting and empowering people affected by Dementia.
You will have:

- A passion for supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives
- A solid and compassionate understanding of the needs of people affected by dementia
- The ability to work inclusively with people affected by dementia, along with an understanding of the need for client confidentially
- Great communication skills, adapting your approach to ensure understanding at all levels
- Effective networking and influencing skills and the ability to manage several tasks at once and keep yourself motivated to perform under pressure
- Good IT skills and experience working with databases and virtual meeting software (e.g., MS Teams and Zoom)
- The ability to travel across Warwickshire area independently when required

Your role will include:

- Supporting people to access vital services
- Improving people’s sense of well-being, enabling them to have more control over their lives
- Assisting with the identification of needs and providing information and access to relevant services
- Promoting our amazing service and building relationships with a range of health and social care professionals
