About the role:
As an Outreach Worker, your role will be to assist your allocated young people in tasks that will enable them to live independently. This support will involve advice, general help and moral support, financial assistance (advice about bill payments etc) and working with young people on completion of education awards (AQA). This role includes travelling to various locations to carry out support sessions with young people.

It will be necessary to attend meetings such as child protection conferences, review meetings and other meetings that are deemed necessary in providing the high-quality service that is expected by local authorities and stakeholders and to ensure service delivery meets young people’s needs. An exciting opportunity has arisen for a Community Development Worker to join our Cambridgeshire team on a Full time basis. The post will be home based with work across Cambridgeshire including advocacy drop ins, advocacy groups and providing training to support people to learn to advocate for themselves. You will begin to develop a team of volunteers that can support some of this work.
You will be promoting POhWER who support a wide range of vulnerable adults often with profound and multiple needs, so experience of networking will be valuable, along with a good working knowledge of public and voluntary organisations.
The successful applicant will be a self-starter with a proven track record in relationship establishment and development and also have experience in community engagement. You will receive a full personalised induction programme and role specific training. Your line manager and supervisor will support you as you develop into your role. A full induction and training schedule will be provided as well as ongoing peer mentoring and support.
Our ideal candidate will have experience in, or a good understanding of, advocacy, information and advice or similar role - ideally in a health, social care or voluntary setting and will have experience of promoting the support of vulnerable adults. More importantly to us, you will need to share our values and passion about advocacy. This is an opportunity to add real value to the lives of vulnerable people.
Because of the nature and location of the role, applicants must live within the advertised area; have a full driver’s licence and access to their own transport. Travel expenses will only be paid within the Cambridgeshire border.

● To empower Young Carers across Brighton & Hove through 1:1 casework interventions to manage their caring role and enjoy a life alongside caring.

Salary scale: £24,000 - £25,000 per annum incl. Outer London Weighting (OLW)
In addition, this role will have a yearly staff retention bonus of up to £2106.
This payment is allocated at the discretion of HMN.
About the Service
This is a pilot working as a Multi-Disciplinary Team (MDT) with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs.
Purpose of Post
Providing advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing. These will be clients who are presenting to District and Borough councils as homeless and need intervention and support with their mental health and remain a source of independent support for all clients. Ensure that the safety and wellbeing of the client is monitored and reviewed regularly and that a person centred recovery orientated and trauma informed approach is embedded approach in all aspects of the roles.
Service Objectives
- To improve the mental wellbeing of people experiencing mental ill health.
- To increase early access to help for people experiencing mental ill health in, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To reduce the use of police, ASB, tenancy enforcement, housing needs and environmental health services by clients whose primary need is related to mental ill health.
- To reduce the use of crisis services by people experiencing mental ill health without positive outcomes for the individual.
- To provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.
- To work collaboratively within an MDT consisting of CGL, District and Borough councils, housing teams and other professionals.
