The Active Wellbeing programme is a specialist mental health exercise referral programme. The programme involves delivering a 10 week, 1:1 personal training programme to clients with severe mental health illnesses (SMI) which can include: Bipolar, Schizophrenia, Severe Clinical Depression.
The Active Wellbeing programme – Learning Disabilities (AW)
The Active Wellbeing programme is expanding to work with adults with learning disabilities who also have a long and enduring mental health condition. The programme LD involves delivering a 10 week, 1:1 personal training programme to clients with long and enduring mental health conditions and individuals with learning disabilities.
Main Duties and Responsibilities:
- Design and deliver a weekly physical activity session for each client considering safety and progression with each of the 10 sessions.
- Monitor clients exercise intensity, type, and ability to ensure clients are exercising safely.
- Monitoring clients mental wellbeing during sessions to ensure safety and communicating any concerns to the line manager.
- Responsible for arranging weekly sessions with clients (and in some cases carers alongside)
- High level of communication between instructor and client (in some cases carers or support workers) to confirm session times and locations.
- High level of communication between instructor and line manager to report any concerns with a clients mental and physical wellbeing during sessions.
- Provide motivation and encouragement for clients to engage in sessions and stay engaged in their programme.
- Responsible for the setup of equipment for outdoor or online sessions.
- Responsible with checking in and out of sessions with the admin team to ensure safety of both instructor and client.
- Responsible for checking the safety of equipment. If equipment is faulty and needs to be replaced, responsible for communicating this with the Active Wellbeing programme coordinator.

Job Purpose
This role is within our crisis alternative service Safe Space - an out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. The service operates 365 days a year from 12:00pm (midday) - 11:30pm across all of our boroughs. The role will consist of two shift patterns to ensure the service is covered: 11:30am-6:30pm and 4:30-11:30pm. This will be on a rota basis across support workers.
Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone on a one-to-one or group basis to individuals experiencing mental health crisis. This will consist of 1:1 support; de-escalating and providing interventions, signposting for specialised or longer-term support, and delivering collaborative self-management plans with clients to improve mental wellbeing. The aim of this service is to de-escalate and prevent further crises.
The objectives of the service include:
- To improve the mental wellbeing of people experiencing mental health crisis in HFEH.
- To provide support to clients accessing the service- for instance: signposting, de-escalation, planned wellbeing activities etc.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To treat service users with respect, dignity and personalised support
- To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation
- To increase self-management skills of those accessing the service
- To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis.
- To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual.
The Role
The role of the support workers will be to support the rest of the team in delivering interventions on a one-to-one and group basis. The role will also involve triaging and assessing all clients accessing the service, signposting and delivering collaborative self-management plans with clients to improve mental wellbeing.

As a Project Worker, you will
- Be involved in undertaking lead working activities such as referrals/interviews/booking in clients and housing management activities such as finances, meaningful activities, health and wellbeing.
- You will assist our clients to develop and care for themselves and build their quality of life by providing advice and guidance on personal development and maintaining their independence.
- You will lead the support in partnership with Care Assistants in washing, dressing and medication, and support with domestic care including shopping and light housework.
- You will also provide high quality comfort for clients in their home.
About you
To succeed, you will either have personal experience of homelessness or a sound understanding of the issues faced by homeless or vulnerably housed people and those with mental illness of substance use difficulties. You should also possess good interpersonal and listening skills, in order to be able to establish an effective working relationship with clients and an eagerness to learn and work with clients in a holistic and person-centred approach.

Join the friendly Carers’ Hub team and make a vital difference to the lives of unpaid carers in Lambeth.
We are looking for two Hospital Carers' Leads to join our new service supporting unpaid carers while the person they look after is in hospital or is discharged from hospital. We know that this can be a worrying and stressful time and that too often carers do not know where to turn to for information and support. Help us make sure that carers get the help and support they need, at this critical time.
You will play a pivotal role in developing and delivering a brand new offer for carers which seeks to increase the identification of carers within local hospitals, improve carer support during their cared for hospital stay and following their discharge from hospital.
Each Carer Lead will be based at Kings' College Hospital (Denmark Hill) or St Thomas Hospital, and will be responsible for:

Offering tailored short-term 1 to 1 support to unpaid carers, working together to develop and implement plans to help improve carers’ experiences during the hospital stay, prepare for discharge and to address carers’ own needs.

Supporting unpaid carers while the person they care for is in hospital via signposting to relevant support and information about their rights.

Working collaboratively with partners within the hospital, to raise awareness of carers and increase identification.

About the project
In an innovative partnership with Southwark Council, Southwark Law Centre, and Impact for Urban Health, we’ve been supporting close to 200 private renters over the last 3 years who have been threatened with eviction by their landlords, or are living in temporary accommodation, to sustain their tenancies and/or find onward housing in the social or private sector that is suitable for their health, needs and aspirations.
About the role
We are looking for an experienced, enthusiastic, and passionate support worker to take up a full-time role within our organisation as a Housing Support Practitioner in our small and dedicated team. The role will be based mostly in Southwark, and you will be working in partnership with the Council’s housing officers, solicitors, community services, social prescribers, and mental health support workers to coordinate tailored support for each client to avoid eviction and sustain tenancies, mediating with landlords where necessary and providing advice for onward housing when needed with the support of the Housing Link Worker. The objectives of these programmes are to avoid evictions, sustain tenancies or move to a better suited property, increase employment opportunities, improve wellbeing, and increase educational outcomes. You will be working in partnership with other stakeholders such as schools and health professionals to improve family health, build relationships and engage families and individuals and their networks of support.

You will have experience of working with individuals with complex needs. You will be an enthusiastic person who is self-motivated, confident and thrives when working independently with a passion to support change. You will have excellent interpersonal skills, experience delivering holistic support, be a solution-minded thinker, have a good understanding of strength-based approaches and be able work sensitively and empathetically with people in vulnerable circumstances. You will also have experience working in a systemic and therapeutic way. It is also important to us that you have the competency to work with people from a variety of cultural backgrounds. To work as part of a team and independently, the roles require you to think creatively and use the resources around you effectively.
