The Community Health and Wellbeing Worker will speak to households about health and wellbeing, alongside offering support to connect people to services and activities in their local community. The role aims to empower people to take control of their lives.
It involves working with people who may be experiencing a number of issues, so you will need outstanding listening and communication skills, empathy and enthusiasm. Main duties include:
- Managing a caseload of households, providing monthly home visits and providing a continuous point of contact for all members of the household.

This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.
The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.
You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We're looking for a Children and Family Worker to join the team in our West London Women's refuge. The service is a refuge accommodation for women and children surviving domestic abuse many of whom are living with trauma. we are looking for someone who really cares about the rights of women and their children.
As a Children and Family worker, you will support in providing a much needed safe and nurturing environment for children and their mother's, providing pathways, support and appropriate interventions to enable resettlement. You will support various teams to co-produce a tailored support plan which is altered to the individual's needs through a recovery model providing trauma informed and enabling environments to achieve this.
Shift Pattern: 37.5 hours per week, working on a rota which is over 7 days including evenings. You will be based on site within the service.

Children and Family Relationships
- Provide parenting advice to support mothers and their families.
- Provide stimulating play opportunities to encourage children from all backgrounds to develop and express themselves, as well as support the children with their general learning, homework, and literacy.
- Support with early learning development, supporting mothers through advice and activity packs which aid their children’s learning and development.
- Deliver family sessions to support contract orders by social services and other organisations, as well as to help rebuild relationships, life skills, confidence, reliance, self-esteem and manage trauma.

- Attend appointments as and when required in support of residents.
- Undertake shift lead responsibilities on rotation, taking responsibility for ensuring day-to-day support delivery is effective, with staff carrying out their designated responsibilities, alongside ensuring resident and service-specific follow up actions are completed.
- Housing Management responsibilities, ensure residents and staff are safe, and the surroundings are kept clean and well maintained.
Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position.
KEY CRITERIA
What we are looking for:
- Understanding and/or experience of working with people of complex backgrounds
- Previous experience and/or in-depth knowledge about working with children who have experienced trauma
- IT Proficiency, ability to learn new software programs, basic Microsoft experience
- Understanding of the trauma children and families may go through due to domestic violence and other previous experiences
- Understanding of the social needs and support required for people with multiple and complex needs
- Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with various challenging backgrounds, mental health, addiction, exploitation, homelessness, and previous convictions
What we would like, but not essential:
- Previous experience creating co-produced support plans, key working, and care planning, providing appropriate interventions for service users
- Previous experience providing housing support and practical assistance within a residential support role

This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.
We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.
Main Responsibilities:
· Undertake 1:1, group, detached and outreach work with young people
· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.
· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.
· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.
· To maintain confidential, up to date and accurate records of all sessions and plans.
· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:
o mental health and wellbeing of children and young people.
o Supporting vulnerable groups who would not otherwise receive support (levelling-up).

· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting

The Active Wellbeing programme (AW)
The Active Wellbeing programme is a specialist mental health exercise referral programme. This programme is designed to engage and promote health and wellbeing to people diagnosed with a severe mental illness (SMI). The programme involves delivering a 10 week, 1:1 personal training programme to clients with severe mental health illnesses (SMI) which can include: Bipolar, Schizophrenia, Severe Clinical Depression.
The Active Wellbeing programme – Learning Disabilities (AW)
The Active Wellbeing programme is expanding to work with adults with learning disabilities who also have a long and enduring mental health condition. The programme LD involves delivering a 10 week, 1:1 personal training programme to clients with long and enduring mental health conditions and individuals with learning disabilities.
Main Duties and Responsibilities:
- Design and deliver a weekly physical activity session for each client considering safety and progression with each of the 10 sessions.
- Monitor clients exercise intensity, type, and ability to ensure clients are exercising safely.
- Monitoring clients mental wellbeing during sessions to ensure safety and communicating any concerns to the line manager.
- Responsible for arranging weekly sessions with clients (and in some cases carers alongside)
- High level of communication between instructor and client (in some cases carers or support workers) to confirm session times and locations.
- High level of communication between instructor and line manager to report any concerns with a clients mental and physical wellbeing during sessions.
- Provide motivation and encouragement for clients to engage in sessions and stay engaged in their programme.
- Responsible for the setup of equipment for outdoor or online sessions.
- Responsible with checking in and out of sessions with the admin team to ensure safety of both instructor and client.
- Responsible for checking the safety of equipment. If equipment is faulty and needs to be replaced, responsible for communicating this with the Active Wellbeing programme coordinator.
- Responsible for administering first aid in the event of an accident and ensuring that safety requirements are met in all areas.

Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. Summary of Role
Working under the direction of our Centre Managers at Arc in the Park and Abbey Hub, Support Workers are key to ensuring the successful delivery of our provision for children and young people. Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.
The Post holder will be expected to work on a rota basis (Monday - Sunday) and two sleep-ins per week which attracts £30.00 extra pay.The shift patterns are 8.00 am - 4.00pm, 2.00 pm - 10.00 pm and sleep ins.
