Job description

An exciting new opportunity has arisen within an established and highly performing service – Full Circle, in Essex. We are looking for 4 x Systems Change Partnership workers. This role has become available due to Phoenix Futures (in partnership with Essex County Council) gaining additional funding through the national Changing Futures pilot. Changing Futures is a £64 million, 3-year long programme, joint-funded between Government and The National Lottery Community Fund. Changing Futures aims to change systems and improve services for people who experience multiple disadvantage, including a combination of three or more of homelessness, substance misuse, poor mental health, domestic abuse and / or contact with the criminal justice system.  Most of these people will not be known to services or not been known in a positive or constructive way. The programme seeks to fill the gaps between services and improve the way local systems work together to support these complex individuals.

We are looking for an individual who has experience of working within the Health & Social care field. The successful applicant will play a key role in building robust working relationships with key partner agencies, as well as using a persistent, consistent and flexible approach to our client group, in order to gain the best possible outcomes. You must have excellent communication skills and an ability to work as part of a team as well as being able to use you own initiative when required and to manage your time independently. We have services across Essex and the location is flexible.

This is a pilot scheme therefore at present the role is fixed term until 31st March 2023 but it is hoped that this pilot will be extended beyond this point once the project is shown to be a success.
