Job description

The Role

DHI is an award-winning charity that challenges social exclusion by supporting people to achieve their potential and contribute to the richness and wellbeing of their community. Are you a positive resilient individual committed to working collaboratively with GP’s, our partners and other community and healthcare organisations to provide a wide range of services including; telephone assessments, content based groups & workshops, creating support plans & risk assessments in a large & mixed team of colleagues and peers?

You feel confident around IT using our Caseload Management IT system to record attendance and outcomes and communicating on the telephone making contact with clients on a daily basis.

You have experience of working with people with substance use issues, and can offer a range of harm reduction advice and information, manage safeguarding concerns and have an interest in developing your knowledge of interventions and support that can in turn support our clients.

Please note, we would especially welcome applicants who speak Polish.
