The Role
Our Practitioner Led Assessment Service is the entry point for our beneficiaries seeking support from the Charity. Our Assessment Practitioners work in partnership with our beneficiaries to identify goals, individual needs and appropriate solutions.
The team provide advice, information and support and work collaboratively with the multi-professional team to ensure our beneficiaries are offered the right support at the right time.
Reporting into the Beneficiary Experience and Continuity Lead you will be responsible for day-to-day management of the practitioner led assessment service, holding a small case load and reviewing and resolving complex cases with the team.
You will work collaboratively with key stakeholders across health improvement and rehabilitation services, to ensure services are integrated, person centred, outcome focused and based on an ethos of recovery.

About You

This is a new role for the Circuit, replacing a volunteer. In designing the position, the Circuit has aimed for maximum flexibility to secure the best candidate for the role. Initially, the person will need physically to attend some meetings to familiarise themselves with the Circuit's operations and ensure a smooth hand-over. For the right person, though, there will be considerable flexibility about how the duties are fulfilled in the longer-term. It is specifically expected that the role will allow for flexible scheduling to accommodate other responsibilities, including caring.
The Richmond & Hounslow Circuit comprises seven churches in southwest London. As with all Methodist circuits, these churches function autonomously yet collaborate closely in their ministry and mission efforts. The Circuit oversees joint resources, including staff and property. Presently, it employs four full-time ordained ministers, one full-time lay pastor, and a part-time administrator. As a part of the Methodist Church, its mission is “to respond to the gospel of God’s love in Christ and to live out its discipleship in worship and mission”. To fulfil this mission, the Circuit fosters a broad spectrum of ministry within its congregations, focusing on worship but also engaging in outreach to the homeless, asylum seekers, new parents, and various other groups. Job purpose
The post holder will be responsible for providing holistic care and support to children and young people (CYP) who have life-limiting conditions and their families as a key member of the community team. This care is provided in the family’s own home, in the community, and support into the hospices and acute settings.
The post holder will be the first point of contact for families following acceptance to the service and the nurse is responsible for undertaking a holistic assessment of the child and family’s needs. The post holder will provide high quality palliative care to CYPs under the care of Shooting Star Children’s Hospices, using their skills and expertise in a diverse range of situations, autonomously and as a member of the community team. The post holder will have excellent communication skills, be a sound educator, and work proficiently.
They will work closely with the SSCH in house team, family support team and symptom management services to ensure a seamless and comprehensive service to CYP and their families.
SSCH care is provided 24 hours a day, 365 days a year, the post holder will be required to work flexibly to meet the needs of planned and emergency care, including on calls overnight and weekends in addition to contracted hours.
Key tasks and Responsibilities
• Providing professional clinical leadership in the provision of palliative care across care settings for CYP under the care of SSCH.
• To work in partnership with the SSCH care teams and wider MDT, demonstrating excellent communication skills to ensure high quality seamless care.
• To ensure that holistic care is family centred, research based, and meets with all statutory requirements and adheres to clinical governance initiatives and best practice.
• Provide support to CYPs and their families where required through community visits to hospital, home or hospice alongside relevant external professionals from statutory services.
• Maintain an awareness of the multi-cultural nature of the SSCH catchment area, adapting care culturally as required.
• To ensure all documentation is maintained in accordance with SSCH policies, guidelines and standards.
• To act in accordance with the SSCH and statutory Safeguarding Children Policy and Procedure.
Clinical
• To be actively involved with the on-going development of the SSCH care service, in particular the community team, contributing to development through research, audit and quality improvement.
• Provide clinical support and continuity of care to CYPs with palliative needs.
• Work alongside specialist symptom management teams to assess symptoms and provide clinical feedback to form part of decision making under the lead of the specialist services.
• Ability to follow an agreed symptom management plan and liaise with teams to offer high level care in the home as needed.
• Working in partnership with all members of the MDT, encouraging and promoting effective team working from all disciplines within SSCH.
• Attend professional meetings as a representative for SSCH and provide proactive input to ensure high level care for CYPs and families.

About Circles South West
We are an award winning independent charity providing services that prevent sexual re-offending by people who have sexually harmed, as well as support services for non-offending partners, family and friends impacted. Could you provide a range of practical and emotional support to improve their knowledge about dementia and their wellbeing?
Age UK Sutton has been commissioned to run a Dementia Support service within the London Borough of Sutton. As a Dementia Adviser you will provide comprehensive, person-centred support and access to information, working closely with other Dementia Advisers and teams across the organisation.
You will work to empower both people living with Dementia and their carers, supporting them to maximise their quality of life. From diagnosis and throughout their journey you will provide access to information and support, help clients to make informed decisions and provide a consistent point of contact. You'll also help to reduce isolation and increase social inclusion by providing opportunities for peer support and raise awareness and understanding of Dementia.
This role is community based working both at our offices and across the Sutton Borough, with some working from home. This exciting programme is currently working with over a hundred fostering services across England, Scotland and Wales, and is looking to grow further. Mockingbird is an evidence-based model structured around the support and relationships an extended family provides. The model nurtures the relationships between children, young people and foster families supporting them to build a resilient and caring community.
We are seeking to grow our team of passionate and dynamic project workers within Mockingbird. You will make a vital contribution helping us to sustain and expand this US based model throughout the UK. In this role you will help make a difference and improve the lives of foster families and of children and young people in foster care. You will provide guidance and support to fostering services to enable them to understand, implement and sustain the Mockingbird model.
This a great opportunity to join the Mockingbird programme at an exciting time of growth in a role that offers the opportunity to have a genuine impact on the experiences and outcomes for looked after children and fostering families.
The role is home based London/East England, but will require extensive travel and occasional overnight stays within the UK on a regular basis.

Assistant CEO responsibilities will include strategic leadership, development, and ensuring the long-term sustainability of our programs. You'll also play a crucial role in promoting EDI and maintaining quality assurance standards.
You will need a professional qualification in mental health or related fields alongside experience in senior management and a passion, resilience and drive to improve the mental health of children and young people.
