Adult Re-settlement Case Worker – Bridge to Home Project
Hours: Full time 35 hours a week
Contract: 12 months
Salary scale: £25,767 per annum, pay award pending
Responsible to: Housing & Wellbeing Lead
Location: Option to work from the office in Chorley or a mix of home and office working, with extensive travel across Lancashire for work activities, meetings, and training.
Bridge to Home is a support initiative aimed at helping people transition from hospital wards back into their local communities. Our goal is to provide a seamless and comfortable experience for those who have been discharged from the hospital and require additional support during their recovery journey.
Your new role
This role will be responsible for delivering personalised intensive one-to-one support and guidance to individuals accessing the Bridge to Home and Lancashire Mind Housing & Wellbeing services in and around Lancashire
You will be required to work closely with hospital ward staff and other statutory services to offer personalised opportunities to people experiencing mental health issues, ready to be discharged from hospital wards.
What you will need to succeed
You will need experience of working with people a diverse range of mental health conditions to overcome barriers to independent living, to have a knowledge of homelessness and housing needs, specific to people with mental health issues and have experience of collaborating with medical professionals, social services, housing providers and local community and voluntary groups.
It is important that the right candidate is an excellent communicator, able to listen and understand people’s needs but also confident communicating and liaising with other local services and partner agencies to ensure each person has the right support in place.
An excellent working knowledge of safeguarding practices for protecting vulnerable people is also key to this role and the successful candidate will need to be able to demonstrate proven experience of this.
Ideally, an understanding of the issues which arise within the social housing sector or experience working in a similar role would be an advantage.
You will need to be a good team player, with the ability to work well with others and be able to work on your own initiative with minimum supervision.

About the role:
This role is based at Whitecross Studios; however, we offer hybrid working. As the Camden Anti-Social Behaviour Project Coordinator, you will be:
- providing emotional and practical support to victims and witnesses of ASB in Camden
- accompanying victims through civil court proceedings, explaining procedure and possible outcomes
- managing the mediation referrals and providing mediation services

About the role:

About the role:
This role is based at our Gloucester office and will require regular travel within the Gloucestershire area. Hence, holding a driving licence is essential.
As a caseworker, you will:

As a Debt & Budgeting Caseworker you will support clients who not only require advice but also have additional support needs, ensuring they engage and remain engaged throughout the advice journey until they have entered a suitable debt solution. About the role:
This role is home working based with travel in the West Midlands area of England. As a Homicide Service Children and Young Person caseworker you will be:

You will be responsible for providing a high-quality, front-line service to bereaved young people following murder or manslaughter. Working with colleagues in the National Homicide Service, as well as partner agencies including education, police, courts, social services and many other statutory and third sector organisations.
You will need to be able to travel to face to face sessions within, and occasionally outside of region, and therefore a driving license and use of a car are essential. You will be reimbursed for all travel costs.

Embedded within the local authority, our successful candidate will carry out one-to-one mentoring work with a caseload of clients (aged 11-25 years) and produce support and risk management plans based on assessments, ensuring that they are equipped to think critically, assess risk, and make better life choices.

The Refugee & Migrant Services Caseworker will join the Refugee & Migrant Services Support Team, who work alongside our ESOL team and volunteers to meet clients’ needs as directed by the R&MS Team Leader. Your responsibilities also include engaging with individuals leading a street-based lifestyle,' including those sleeping rough, with the objective of connecting them with the appropriate services and facilitating their transition into more stable living conditions. You'll work to increase service user choice, involvement, and empowerment, promoting sustainable change while coordinating efforts to achieve positive outcomes for each client. Additionally, you'll focus on building and strengthening service user's personal strengths and social networks, both in the UK and their country of origin.
This role demands flexibility and a proactive approach to engage with hard-to-reach service users and provide a range of effective interventions. Key activities include identifying and establishing contact with service users sleeping rough, including those initially unwilling to engage with services. You'll collaborate with drug and alcohol teams to facilitate access to treatment, work with Mental Health Agencies to address mental health issues and ensure ongoing health care access for service users. Moreover, you'll support efforts to divert newcomers to rough sleeping, encourage people to return to their home communities, and cooperate with local Housing Providers to maximise accommodation opportunities.
