We are working with the SWL ICB and local GPs to deliver an exciting new service within Hampton North. The Community Health and Wellbeing Worker will speak to households about health and wellbeing, alongside offering support to connect people to services and activities in their local community. The role aims to empower people to take control of their lives.
It involves working with people who may be experiencing a number of issues, so you will need outstanding listening and communication skills, empathy and enthusiasm. You will need excellent organisational skills, be adept at seeking out information for relevant community services and have the tenacity to help people achieve sustainable improvements in their wellbeing.
Main duties include:
- Managing a caseload of households, providing monthly home visits and providing a continuous point of contact for all members of the household.
- Working with the client to assess their needs, create personalised action plans and locate support within the community.
- Supporting clients to prioritise and address practical, health, social and emotional issues in their lives which affect their wellbeing.
- Engaging with community organisations and specialist services that provide support to ease the pathway for your clients.
- Building and maintaining relationships with primary care networks, GPs, health care professionals and services across the borough.

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing.
You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. At the heart of Emmaus Greenwich is our companions. Companion is the name given to those who live in an Emmaus community and contributes to the social enterprise, where they support themselves and one another. OVERVIEW OF KEY RESPONSIBILITIES
- Set up realistic and flexible strengths-based support plans, working closely with colleagues and support teams to support and advise on interventions and approaches to meet resident/participant needs.
Include appropriate substance misuse interventions within support plans for those with co-occurring substance dependence and mental health need devising systematic plans for care; implementing this plan; coordinating the input of others and evaluating effectiveness. - Undertake joint holistic assessments and risk assessments with residents/participants to help determine service and support needs and identify where these can be met. Provide outcome-based support measured against goals, recovery, and achievements.
- Encourage residents/participants to identify and define their own goals and work towards being resilient and independent as possible, support with reducing reliance on clinical interventions and hospitalisations.
- Promote active involvement of those with dual diagnosis, assist residents/participants to identify their own strengths, personal interests and goals and support them to access and participate in activities of their choice.
- Undertake time-limited, SMART, and a range of evidence-based interventions such as 1-1’s, including harm minimisation, increasing awareness and motivation through stages of change and relapse prevention.
- Support in creating and maintaining a welcoming, psychologically informed home environment for residents/participants, ensuring a continuous focus on safety, health, and wellbeing.
- Positively promote and support connections with local communities and boroughs of SIG’s services and liaise with other teams to maintain and share knowledge and links to support people to access resources which include print, personal and digital.
- Develop community links and liaise regularly with a range of agencies and individuals across voluntary and third sector services in the community including but not limited to; drug and alcohol services, supported employment schemes, gyms and healthy living programmes, recovery colleges, and benefits and advice agencies.
- Act as a role model for staff of all disciplines and grades working with people with co-occurring mental health, alcohol and/or drug problems.
- Share knowledge and skills to help enhance awareness around dual diagnosis and best practice, challenges, and services available with other services of Social Interest Group.

KEY CRITERIA
What we are looking for:
- Knowledge, understanding and experience of working with people of complex backgrounds. Ideally ex-offenders, those with enduring mental health, substance misuse and challenging behaviours
- Experience of assessing needs and strengths, and supporting people with dual diagnosis and complex needs
- Previous experience in a similar role, providing holistic assessments and support for people experiencing mental illness who use substances
- IT Proficiency, including Microsoft Office, and the ability to navigate and learn new case management systems and other types of organisational software
- Able to recognise ill health and additional support and need for interventions
- Knowledge of the interactions between mental health problems and drug and alcohol use and the issues facing people trying to stabilise their situation
- Applied knowledge of the principles of risk assessment and risk management, particularly in relation to harm minimisation

Regional Youth Worker - London
Salary: £27,000
Location: Flexible, based at our office in Aldgate East, with travel throughout the London region and occasional remote working.
We are looking for an experienced Youth Worker, to deliver training sessions and projects to young people through youth centres, community groups, schools and alternative provisions across the London Region, and to ensure our programmes are fit for the future.
We are an award-winning national charity which trains over 9,500 young people affected by street violence each year in emergency first-aid through a team of 350 healthcare volunteers and youth workers. We empower young people with the skills to become part of the solution to violence by developing the skills and confidence to save lives and increasing their understanding of the medical and psychological consequences of violence.
As the StreetDoctors Youth Worker in the Region, you will deliver high quality training, develop new partnerships and maintain positive relationships.
The key areas of focus include:
● To deliver training sessions to young people and partners to required quantity and quality, and support the development, testing and implementation of new training content.
● To create, develop and maintain partnerships with a range of organisations including, funders, commissioners, community organisations, criminal justice organisations, education providers and youth organisations.

This is an exciting opportunity to be part of and help shape a new DHSC funded project - Early Support Hubs - providing innovative ways to support Young People’s (11 – 25 years) Mental Health and Wellbeing across the borough of Hackney.
This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.
We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.
Main Responsibilities:
· Undertake 1:1, group, detached and outreach work with young people
· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.
· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.
· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.
· To maintain confidential, up to date and accurate records of all sessions and plans.
· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:
o mental health and wellbeing of children and young people.
o Supporting vulnerable groups who would not otherwise receive support (levelling-up).
o Improving children and young people’s engagement with EET.
Main Requirements (for details check the job description and person specification):
· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting
· Experience of working with young people, delivering therapeutic groups and/or 1 to 1 interventions in a youth work or similar setting.
· To have an active interest in working with those affected by mental health issues and recognise the impact this has on their life.
· Experience of building positive, impactful relationships with young people responding to their needs
· Excellent, up to date knowledge and understanding of safeguarding needs and risks young people may face.
· Experienced in working as part of a safeguarding team around a child, i.e. Childrens services, education, CAMHS, etc
· Experience of working in with an outreach role, in the community and school settings.
· Confident working well independently as well as in a team setting.
· Ability to use own initiative and be able to respond to change as part of a pilot initiative.
· Computer literacy including Microsoft Office suite (incl. Outlook and Word), with ability to accurately input sensitive data electronically

Are you passionate about improving support for families affected by substance misuse?
This could be the perfect role for you! The Family Support Worker is an exciting role within a growing and dynamic team, where you can help to make a real difference to the lives of unpaid carers in Lambeth.
Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project.
The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.
The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

Carers’ Hub plays a vital role in Lambeth, supporting the many unpaid carers across the borough. Our work stems from the fact that carers often go about their roles despite the toll it might take on their own wellbeing, often having to make sacrifices that hamper their own ability to lead a normal life. Whether financial, educational or otherwise, we seek to limit the challenges that carers face. We achieve this through four core workstreams:
