What you’ll be doing? 

The Women’s Gambling Link Worker will deliver an awareness-raising and personalised, holistic support programme for women in Lambeth experiencing gambling and gambling addiction-related harm, or those impacted by the gambling of a close family member. You will:

• Deliver an outreach programme by regularly attending community events, visiting local groups/ spaces and organisations frequented by women in Lambeth.

• Develop partnerships and deliver outreach support to primary and secondary care partners and services across Lambeth.

• Speak with women's groups in Lambeth, either 1-2-1 or presenting to groups, to discuss the type of support available and promote the work and support of the Primary Care Gambling Service and Age UK Lambeth services. 

• Provide holistic support to patients referred to the Primary Care Gambling Service, helping to address housing, debt, bereavement, loneliness, mental health and other related social issues, that are both a symptom and/or a cause of their gambling.

• Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss ‘what matters to me’.

We have an exciting new opportunity for a Social Prescribing Link Worker to join our team in Merton.

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing. 

You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.

In the role of Specialist Multiple Disadvantage Worker, you will be expected to work in conjunction with the Team Manager to manage all referrals and assessments in a timely manner to ensure all assessments are conducted in a skilled and sensitive way to encourage potential clients to openly discuss their strengths as well as their deficits.  

As a Specialist Multiple Disadvantage Worker, you will be responsible for the delivery, and evaluation, of a range of Life Skills, and Recovery workshops and activities to support our clients' skills acquisition. To give support and guidance to project workers in relation to best practice standards for supporting homeless people with multiple disadvantages and complex needs.

The Specialist Multiple Disadvantage Worker will have skills in effectively liaising and building good relationships, with external and internal stakeholders to keep a satisfactory level of appropriate referrals.

About you:

• An understanding of best practices in supporting people with complex needs and an ability to support and guidance colleagues.  

• The ability to effectively and intensively case management a small case load of client with complex needs. Also, can create SMART support plans and effectively assess risk.  

• Experience and commitment to working flexibly and creatively to response to changes in sectorial best practice, external environment, organisational requirements, etc.  

• Experience of working with or supporting vulnerable people with support needs around any of the following issues; rough sleeping, mental ill-health, physical health, neuro diversity, problematic substance misuse, ex- offending.  

• To have a non- judgmental approach to working with clients who have multiple disadvantages, to promote a strength –based approach and an understanding of and commitment to Equity, Diversity, and Inclusion.

ABOUT THE ROLE

A new opportunity has arisen to join a new contract in Lewisham. You will support over 2 properties which supports residents and participants with acute and/or complex mental health needs who are supported/treated in the community in their recovery as appose to hospital inpatient care. You will focus on rehabilitating and reintegrating our residents and participants and help our residents and participants achieve their personal goals and gain a level of recovery that enables them to return home or move into supported housing.

Shift Pattern: Rota based Monday to Sunday between 08:00 and 19:30 

ABOUT THE SERVICE

The Integrated Community Rehabilitation Services will enable people to receive a community-based offer and avoid admission into a rehabilitation unit. 

The service is across 2 properties in Lewisham (one 8 self contained rooms, and another with 6 self contained rooms) located in close proximity to each another and both will be staffed 24/7 by a minimum of 2 Mental Health Recovery Workers (MHRW) at all times, at each site. This will include Shift Leads to support the Management Team. 

These addresses are a short walk from each other and are served by good public transport links; close to Bellingham, Catford and Catford Bridge train stations, and close to bus stops.

KEY CRITERIA

What we are looking for:

• Previous experience working with people with multiple complex backgrounds/needs, mental health, substance misuse, alcohol, and behaviours 
• Previous experience creating co - produced support plans, key working, and care planning, providing appropriate interventions for service users
• IT Proficiency; ability to learn new software and basic Microsoft
• Understanding of the housing and social needs of people with multiple and complex needs
• Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with various challenging backgrounds, mental health, addiction, exploitation, homelessness, and previous convictions

What we would like, but not essential:

• Understanding and/or practical application of key legislation regarding social care, housing, criminal justice, and mental health
• Ability to Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent crisis

This is a new role to the team and you will deliver evidence-based interventions based on  holistic assessments to support personalised recovery, integration, and promotion of independent living skills. You will work to the long term support needs of our residents and work towards facilitating improved outcomes. 

Our service has 27 residents across 4 sites, which specialise in Forensic Mental Health aiming to enable and empower our residents to achieve their maximum levels of functional independence and enhance their sense of wellbeing.

Shift Pattern:  This role is full time 37.5 hours per week. Hours to be discussed, you will be based in service.

OVERVIEW OF KEY RESPONSIBILITIES

• Set up realistic and flexible strengths-based support plans, working closely with colleagues and support teams to support and advise on interventions and approaches to meet resident/participant needs.
  Include appropriate substance misuse interventions within support plans for those with co-occurring substance dependence and mental health need devising systematic plans for care; implementing this plan; coordinating the input of others and evaluating effectiveness.
• Undertake joint holistic assessments and risk assessments with residents/participants to help determine service and support needs and identify where these can be met. Provide outcome-based support measured against goals, recovery, and achievements.
• Encourage residents/participants to identify and define their own goals and work towards being resilient and independent as possible, support with reducing reliance on clinical interventions and hospitalisations. 
• Promote active involvement of those with dual diagnosis, assist residents/participants to identify their own strengths, personal interests and goals and support them to access and participate in activities of their choice.
• Undertake time-limited, SMART, and a range of evidence-based interventions such as 1-1’s, including harm minimisation, increasing awareness and motivation through stages of change and relapse prevention.
• Support in creating and maintaining a welcoming, psychologically informed home environment for residents/participants, ensuring a continuous focus on safety, health, and wellbeing.
• Positively promote and support connections with local communities and boroughs of SIG’s services and liaise with other teams to maintain and share knowledge and links to support people to access resources which include print, personal and digital.
• Develop community links and liaise regularly with a range of agencies and individuals across voluntary and third sector services in the community including but not limited to; drug and alcohol services, supported employment schemes, gyms and healthy living programmes, recovery colleges, and benefits and advice agencies.
• Act as a role model for staff of all disciplines and grades working with people with co-occurring mental health, alcohol and/or drug problems. 
• Share knowledge and skills to help enhance awareness around dual diagnosis and best practice, challenges, and services available with other services of Social Interest Group.

Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position. 

KEY CRITERIA

What we are looking for:

• Knowledge, understanding and experience of working with people of complex backgrounds. Ideally ex-offenders, those with enduring mental health, substance misuse and challenging behaviours
• Experience of assessing needs and strengths, and supporting people with dual diagnosis and complex needs
• Previous experience in a similar role, providing holistic assessments and support for people experiencing mental illness who use substances
• IT Proficiency, including Microsoft Office, and the ability to navigate and learn new case management systems and other types of organisational software
• Able to recognise ill health and additional support and need for interventions
• Knowledge of the interactions between mental health problems and drug and alcohol use and the issues facing people trying to stabilise their situation
• Applied knowledge of the principles of risk assessment and risk management, particularly in relation to harm minimisation
• Ability to engage those who may be hard to reach, able to influence and negotiate positive outcomes with others

What we would like, but not essential:

• Experience in delivering training in motivational interviewing, Cognitive Behavioural Theory informed approaches and other relevant recovery models

Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. The Dementia Support Worker is responsible for managing a caseload of individual clients, providing practical and emotional support to the most isolated and vulnerable in our community. The support takes the form of weekly or bi-weekly visits to the client’s home, as well as contact by phone and email and liaising with social care and health professionals as directed by the client’s wishes or needs.

We are seeking a Part Time Dementia Support Worker (up to 21 hours per week)

Bonny Downs Community Association has been working together to change lives and transform our community since 1998. Over the past 25 years our team has grown to provide a wide array of services that benefit Newham residents from all walks of life. 

This role is specifically funded through the Youth Endowment fund as a pilot project ‘Thriving Communities’. You will be required to work alongside Thriving Communities commissioners, as well as the project’s Youth Justice Worker’s, Educational Psychologist, and/or professionals assigned to individual young people’s cases- such as teachers, social workers and mentors. The Thriving Communities project is a small part of a wider collaborative project, EMPOWER YOUTH, delivered by two partner charities so, although this position is based at BDCA, you will be working closely with a second East Ham Youth Partnership Project lead and team of Sessional Assistants based at nearby charity The Renewal Programme (TRP). Staff from each organisation will work together to deliver the programme and cross-refer young people to activities led by both teams. You will also be required to work with BDCA’s own Children & Families team during the summer scheme and liaise with our marketing team all year round. External partner agencies that you will work directly with include Thriving Communities panel, Newham Youth Empowerment Service, Newham Community Engagement Team, local sports clubs and schools.

Are you passionate about improving support for families affected by substance misuse?

This could be the perfect role for you! The Family Support Worker is an exciting role within a growing and dynamic team, where you can help to make a real difference to the lives of unpaid carers in Lambeth. 

Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project. 

The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

Carers’ Hub plays a vital role in Lambeth, supporting the many unpaid carers across the borough. Our work stems from the fact that carers often go about their roles despite the toll it might take on their own wellbeing, often having to make sacrifices that hamper their own ability to lead a normal life. Whether financial, educational or otherwise, we seek to limit the challenges that carers face. We achieve this through four core workstreams: 

1. raising awareness of carers, 

2. influencing local policy through community engagement activities, 

3. improving carer wellbeing and

4. connecting carers to each other and to support and training opportunities through our services.
