The St Mungo’s Pan London Rough Sleeping Teams (No Second Night Out and Somewhere Safe to Stay) provide a rapid response service, intervening to ensure no one needs to sleep rough. We are currently looking for people to join our teams as Night Assessment Workers in our No Second Night Out Assessment Hubs. In these roles you will:

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. We are recruiting Waking Night Support Workers to join our dynamic service and team. In the role of Waking Night Support Worker; individuals will ensure continues support is provided to young people at the standards of day staff with particular focus on safety of vulnerable young people.
About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. The role requires individuals to work on a rota pattern including weekend and bank holiday working, as the service is open 365 days a year. Hours worked: 21:30 - 07:30.

Overview
The Social Mobility Foundation is seeking to recruit group leaders to support and supervise over 300 Year 12/S5 or Year 13/S6 students on internship programmes this summer. The young people will be high-achieving students from low-income backgrounds and they will be undertaking internships in the following professions: Banking and Finance, Business, Engineering, Law, Digital and Medicine. The internships will take place in person in either London, Middlesbrough, Manchester or Glasgow.
Accommodation: The group leaders will be provided with accommodation in the same location as the participants on the programme and will be responsible for the welfare of the young people staying there. The dates of the programmes running are listed below.
Training: All Group leaders will be required to participate in training provided by the SMF before the residentials begin and then adhere to all aspects of that training. Day Group Leader Hours: 7.30am-10am and 4.30pm-10.30pm. Leaders are required to be ‘On-Call’ (excluding breaks) between 10am-4:30pm and may be asked to do additional work during these hours. Leaders will be paid for any additional hours worked.

This is an exciting opportunity to join our team in Worcestershire. We’re looking for an enthusiastic, motivated and well organised individual to join us in delivering a comprehensive Stroke Recovery Service across the region.
Position: S11158 Stroke Association Support Coordinator
Location: Home-based, Worcestershire, however, extensive travel will be required as part of this role (may include team meetings or other work-related meetings)
Hours: Part-time, 24 hours per week
Salary: Circa £17,546 per annum (FTE circa £25,500 per annum)
Contract: This is a fixed-term contract until 31 March 2025. Our services are contracted, we currently have funding for this contract until 31 March 2025.

The Role
Reporting to the Stroke Association Service Delivery Coach, the coordinator will work with the local Health and Social Care services to receive referrals and support stroke survivors and their carers. As a coordinator you will provide personalised information, advice and support with regular reviews throughout.
Key responsibilities will include:
· Working with the local Health and Social services to receive referrals to support stroke survivors and their carers
· Providing personalised information, advice and support to address any needs identified
· Completing Needs Assessments with stroke survivors and supporting them to develop a stroke recovery plan
· Providing regular reviews to support people in establishing and achieving their own personal goals.
· Completing 6-month Post Stroke Reviews
About You
You will:
· Have a background in a caring and/or charity profession supporting people with disabilities
· Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
· Have experience of working with people who may have additional communication support needs
· Have the ability to use basic Microsoft system
This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.
