MYcommunity Lambeth works in partnership with a large number of statutory and community services and organisations across the borough. You will link clients with appropriate services to meet their needs. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing.
You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. This could be the perfect role for you! The Family Support Worker is an exciting role within a growing and dynamic team, where you can help to make a real difference to the lives of unpaid carers in Lambeth.
Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project.
The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.
The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

· Undertake 1:1, group, detached and outreach work with young people
· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.

The Rape and Sexual Abuse Support Centre (Rape Crisis South London) is looking for a committed, professional and skilled Helpline Support worker to provide high quality emotional support, information and signposting in our Support Line team.
The 24/7 Rape and Sexual Abuse Support Line is a partnership between Rape Crisis England and Wales, the Rape and Sexual Abuse support Centre (RASASC - Rape Crisis South London) and two other Rape Crisis Centres (ARCH Teesside and Lincolnshire Rape Crisis). The post-holder for this role is employed by RASASC (Rape Crisis South London).
The post-holder will provide emotional support, information and signposting to survivors of any form of sexual violence, of any gender, aged 16 and upwards, and their supporters via phone calls and chats. Helpline Support Workers also provide support to colleagues and volunteers as part of their daily duties. Helpline Support workers have experience of either providing emotional support on a telephone helpline or experience providing support to survivors of sexual violence. You will also need to have good knowledge and understanding of the impact of sexual violence.

We're seeking a skilled and dedicated individual to join our team as a Young Person’s Worker. In this role, you will play a vital role in providing support to at-risk young people who are experiencing homelessness.
Your mission: to support and empower young people to develop the lifelong tools they need to lead fulfilling lives outside the confines of traditional services. You will carry a caseload of clients who you will formulate a comprehensive support and safety plan with, which forms part of the ongoing casework, promoting independence and preparing young people for moving into independent accommodation, and employment and education.
About the Role:
As the Young Person's Worker, you will be working within a strengths and recovery framework embedded within a trauma informed approach to ensure client needs and aspirations are at the centre of the support planning process. You will be the lead worker overseeing the support and development of the young person’s needs and aspirations. You will guide and signpost the young person to the relevant statutory and non statutory services and will be key in motivating and coaching the young person to develop independent living skills.

Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. In some services, this will include providing physical, domestic, emotional and social care. What you'll do:
If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. In some services, this will include delivering personal and physical care as appropriate
Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
Participate in the support planning and risk management, as facilitated by the lead Support Worker
Carry out security duties to ensure the safety of the customers and premises
Monitor the CCTV throughout the shift
Enable customers to make full use of community facilities by providing support as directed
Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities
Report any observations relating to customers welfare
Adhere to all the policies and procedures of Look Ahead Care, Support & Housing plus those specific to the project including any statutory requirements
Ensure Look Ahead Health and Safety policies and local protocols are adhered to at all times
Maintain records as required at the project under the direction of the Support Workers/ management
Participate in team meetings/reviews and the general development of the service
Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned

We are looking for a dynamic, enthusiastic and highly motivated individual with experience in administration within the social care field to work with us as a Family Finding Worker. This role is home-based, and it will cover placements for our Yorkshire & North-East areas.
The Family Finding Worker is responsible for receiving placement requests for children who need to be looked after and matching them to our foster carer in cooperation with the Duty Social Worker. Successful candidates will be given the opportunity to join a busy, dynamic and engaging team working in Newham. Support Workers enable people with particular needs to manage their tenancies and live independent lives of their choosing in the community and move towards greater independence by providing a flexible and individually tailored support package. Support Worker will give practical support to help people maintain their tenancy, stay well, exercise choice and control in their care and lives and participate in the community. Support is also provided to develop a model of re-enablement towards self-advocacy, self-management and empowerment, incorporating the principles and practice of recovery, co-production and peer support.
The service includes four sites and form a mini pathway of higher, medium and move on support accommodation.
The shift pattern for this role consists of three shift timings at: 9.00 - 17.00, 12.00 - 20.00 and 14.00 - 22.00. This includes weekends and bank holidays.
