The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing. 

You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.

This could be the perfect role for you! The Family Support Worker is an exciting role within a growing and dynamic team, where you can help to make a real difference to the lives of unpaid carers in Lambeth. 

Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project. 

The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

This is an exciting opportunity to be part of and help shape a new DHSC funded project - Early Support Hubs - providing innovative ways to support Young People’s (11 – 25 years) Mental Health and Wellbeing across the borough of Hackney.

This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.

We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.

Main Responsibilities:

· Undertake 1:1, group, detached and outreach work with young people

· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.

· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.  

· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.

· To maintain confidential, up to date and accurate records of all sessions and plans.

· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:

o mental health and wellbeing of children and young people.

o Supporting vulnerable groups who would not otherwise receive support (levelling-up).

o Improving children and young people’s engagement with EET.

· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting

· Experience of working with young people, delivering therapeutic groups and/or 1 to 1 interventions in a youth work or similar setting.

· Experience of building positive, impactful relationships with young people responding to their needs

You will be the parent or carer of a child or young person with Autism.

You will provide an advice, information and guidance service to predominantly Black, Asian and minority ethnic (BAME) and East European families in the London Borough of Ealing who have been referred to the All Age Autism Hub for assessment.

You will respond to first enquiries and contribute to the Ealing All The Autism referral hub service development and workings of the Hub as an expert by experience.

You will support group sessions for the Autism Hub as well as any of the Ealing run projects as and when required. You will support the Hub clinical team and Contact Ealing colleagues with administrative support around group sessions and training for families and professionals.

You will assess families’ needs and plan actions with parents’ participation as per their needs. You will encourage parent carers’ empowerment and involvement in the project.

We are seeking an Advanced Recovery Practitioner to support in the Women and Families team, within Mind in the City, Hackney and Waltham Forest, working in partnership with Turning Point and Antidote.  The successful candidate must have demonstrable experience in working within a substance misuse treatment and recovery setting, must have specialist training and supervision in one or more complexities alongside substance misuse. This may be mental health, long term conditions, criminal justice, offending behaviour, homelessness, motivational interviewing, safeguarding, family work.  Have an understanding of risk and risk management, as it applies to substance misuse and complexity.  Have a working knowledge of Safeguarding of Vulnerable Adults framework, the ability to follow procedure and development of a culture of safe practice.  Ability to work under pressure and have significant experience of working in health and social care.

You will receive support and training and a competitive starting salary of £20,000 for a 28 hour week. Reduced hours may be possible by agreement. This is a two-year fixed term post with the possibility of becoming permanent after two years.

We are looking for someone who will work with professionalism, empathy and kindness, have knowledge of working with children and families who have experienced trauma due to abuse and have a trauma informed, child focused approach. Experience of working with complex family situations where there are safeguarding concerns with the ability to assess risk is required.

Right now, our Croft House Supported Housing service in Surrey is looking for Recovery Workers to support the people who use our services to maintain their tenancy and independence in the community. That’s why we need you to inspire and support them to lead their own recovery. 

So, whether it’s helping to come up with a personal support plan that will see them achieve their goals and aspirations, developing domestic and finance management skills or accessing work, leisure or educational opportunities, your empathy, enthusiasm and compassion will have every chance to shine. Entering accurate case notes onto our IT system will be important too. And, when it comes to regularly liaising with GP surgeries, the local council, charities etc. or promoting the service within the community, again, we'll count on you.

No relevant experience is required as full training will be provided, although candidates with an understanding of mental health issues and/or lived experience would be welcomed. More important is your caring and compassionate nature and empathy and enthusiasm for helping others. Whatever your background, you’ll need to be happy to work both independently and within a team and willing to be part of a weekly rota system.

Richmond Fellowship is part of Recovery Focus, a national group of charities highly experienced in providing specialist support services to individuals and families living with the effects of mental ill health, drug and alcohol use, gambling and domestic violence. Dementia Wellbeing Worker 

Age UK Camden Dementia Wellbeing Service is looking to recruit a Dementia Wellbeing Worker to join their team working 35 hours per week on a permanent basis. 

The Dementia Wellbeing Service is part of our specialist dementia services and works closely with the Dementia Befriending Service.

The service supports Camden residents over the age of 55 in the early or moderate stages of dementia, to live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love.  

The successful candidate will: 

• Offer a holistic and person-centered assessment for people living with mild cognitive impairment or Dementia to ensure meaningful interventions.

• Manage a case load of clients, providing practical and emotional support to the most isolated and vulnerable in our community.

• Provide information, advice and support to carers (family/friends), health/social care professionals and other voluntary organisations and to disseminate knowledge and best practice so that individuals living with cognitive impairment in relation to dementia or undiagnosed dementia have the best care possible.  
   

• Maintain accurate and appropriate records via Charity Log database and in accordance with UKGDPR. 

Salary: £33,525.36 to £34,743.24 pa incl. London Weighting.
