Newham Clinical Commissioning Group’s (NCCG) vision is for Newham Community Prescription (NCP) to recast the culture of Newham so that health and resilience replace fatalism and health services promote health as well as manage sickness. The NCP will become a fully integrated primary care patient pathway that will reduce health inequalities by creating a community infrastructure that connects people with the resources they need to be healthy and supports them to manage the non-medical but health critical social determinants that influence their health status and compliance with recommended treatments.
Phase one of NCP, planned for September 2013 to March 2014, aims to prevent or delay the onset of diabetes by equipping local primary care clinicians with ‘prescribing solutions’ that address the wider social determinants of health by creating a community infrastructure that will connect patients to the resources they need to enable and empower them to better manage their health and improve their wellbeing.
The post holders will assist with the development, implementation and sustainability of the NCP by developing and facilitating a flexible client-centred approach, linking patient referrals to accredited Communities of Health (COH) and other organisations for a wide range of programmes, activities and support. This will be aimed at enabling individuals to develop their knowledge and skills about health and wellbeing and change their behaviours to improve their health.
The scheme will adopt an evidence based research approach, involving base line assessment strategies and outcome measures (for both patients and organisations) through a monitoring and evaluation system to inform NCCG, GPs, stakeholders, partners and (COH) organisations of the viability and sustainability of the NCP.
It is expected that up to 1000 patients will be referred by GPs for physical activity in a participating COH organisation (to allow for a minimum of 250 participants). Each patient will be expected to attend 12 week sessions upto 3 times a week, in one/several organisation/s.
The post holder will be expected:
• To assist in setting up, delivering, coordinating and evaluating Phase 1 of NCP until March 2014
• To liaise with GPs, COH groups and other stakeholders to ensure the successful implementation of NCP
• To support COH partners to participate in NCP effectively and deliver appropriate activities that comply with agreed standards
• To help with the daily management of administration of the project and contribute to project management and planning processes
• To collect monitoring, evaluation and other relevant data and contribute to keeping accurate and comprehensive records
The main duties of the post holder will be to:
• Establish and maintain links with local COH organisations and other relevant groups and activities within Newham and liaise with them to arrange ‘placements’ for referred patients
• Work with such existing and new COH organisations to maximise relevant provision and patient referrals, and provide ongoing support to COH staff and other partners to ensure successful implementation of NCP
• Respond promptly and effectively to GP referrals
• Undertake one-to- one/small group interviews with referred patients and deliver patient assessments to identify health concerns and motivation
• Assess individual patient needs according to agreed criteria and make appropriate referrals to respective COH organisations after exploring different available options
• Signpost patients to other services as relevant
• Help referred patients access and use COH activities by setting up introductory meetings and induction and/or accompanying where necessary, referred patients to COH/other organisations, as appropriate
• Work with local GP practices and primary care clinicians to promote and facilitate the NCP
• Help build up and maintain knowledge of contacts, support and resources within community using appropriate retrieval systems
• Keep adequate records of the work undertaken as agreed with the team leader/line manager
• Work closely with the other navigators ensuring good communication and consistent, high quality practice and joint problem-solving as required
• Share information about emerging good practices/ issues/barriers to engagement and ways in which NCP can be improved to better the health and wellbeing of local people.
Salary Scale PO1 point 33 £29,604 pro rata including Outer London Weighting
Hours: Two Part-time posts, 3 days per week, some evening/weekend work as required
Duration:up until 31st March 2014 (with possibility of continuation)
