Location: Hybrid role with Nominal office working in Chorley, and regular travel to venues around Lancashire for tenant visits, training, and team meetings.
Lancashire Mind’s Tenancy Management Service works in partnership with several registered housing partners across Lancashire to manage a portfolio of properties on their behalf and provide tenancy support to people living with moderate mental health conditions.
We care about making a difference to people’s lives and through the provision of regular support, we work with each person to improve their mental wellbeing and build the confidence and skills to sustain their tenancy and move on to independent living.
We use an outcome-based approach, making sure that each person is involved in their own support planning from the start of their journey with us and achieve this by offering encouragement and one to one support to help each person to understand and manage their tenancy, develop social and daily living skills, gain access to essential services to manage their mental and physical health and help and guidance to maximise their benefits and improve their economic wellbeing.
Your new role
We are looking for a new Tenancy Management Worker to join our small and friendly team to manage a caseload of customers who have complex needs including mental health conditions, substance misuse, homelessness, and a possible history of offending.
Your job will be to work closely with these individuals to promote independent living and help them to achieve and sustain their tenancies. This is a challenging and varied job which is often rewarding, particularly when the people we have supported achieve their own goals and move on to build better lives.
What you will need to succeed
You will need experience of working with people with complex needs ideally in a supported or social housing setting and have the resilience to support people with a diverse range of mental health conditions to overcome barriers to independent living.
It is important that the right candidate is an excellent communicator, able to listen and understand people’s needs but also confident communicating and liaising with other local services and partner agencies to ensure each person has the right support in place.
An excellent working knowledge of safeguarding practices for protecting vulnerable people is also key to this role and the successful candidate will need to be able to demonstrate proven experience of this.
Ideally, an understanding of the issues which arise within the social housing sector or experience working in a similar role would be an advantage.
You will need to be a good team player, with the ability to work well with others and be able to work on your own initiative with minimum supervision.
This is an essential car user role as it requires extensive travel to and from tenants’ properties in Lancashire.

You will support victims of domestic abuse within the Stockport Borough offering a person centred and trauma informed package of support and advocacy.
The role
In this role you will complete risk and needs assessments with victims of domestic abuse who live in the borough of Stockport. You will liaise with referring agencies, actively contribute to risk management procedures and team working. You will carry out a large and varied range of practical support which may include safety planning, advocacy, risk assessment and risk management. We also offer emotional support to those people we work with, encouraging and supporting them to rebuild lost confidence and self-esteem. We are a consent driven service and therefore the needs of the person we are supporting is at the core of all the work that you will do.
As part of the Stockport Domestic Abuse Support team, you will also be supporting the partners of those who are undertaking behaviour change work on the Striving to Change Programme.
You will attend regular risk management meetings to ensure that both parties are fully supported and that the risk is appropriately managed.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. Lincolnshire Crisis Service have two houses in the county, one in Boston and another in Lincoln, we provide a range of support for people with mental ill health and both work to a recovery model. We have been operating in the area for a number of years and offer a staffed service to our residents aged 18 and upwards. This role does require you to work varied hours as the service is 24/7 and may include some sleep in shifts, these are paid in addition.
We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in.

About this role…
Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
Thames Reach welcomes applications from people with transferable skills.
You will:
• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping across four districts (Mole Valley district council, Tandridge district council, Epsom and Ewell Borough Council, Reigate and Banstead borough council).
• Support our clients with practical tasks that will enable them to move off of the streets or sustain accommodation.
• Directly support clients, identifying activities that will assist in their journey towards recovery and away from life on the streets.
• Maintain and support links with community programs and resources.
• Ensure that all paperwork is completed and work is recorded and monitored.

Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. This role is based within our team in the East of England. As a Support Worker, you will provide exceptional housing support services and housing management services to a diverse range of service user including those on Bail, prison leavers, and those going through the criminal justice system.
The Team provides flexible and holistic support to residents with a wide range of support needs. We aim to support them to find and keep a stable home at the end of their CAS-2 placement. You will manage a caseload of clients in properties in your allocated patch in the East of England.

Key Responsibilities
- Plan and deliver effective support and safety plans with the service user referring to specialist agencies as required.
- Ensure that properties are always adequately equipped, maintained, furnished, and cleaned.
