Job description

Age UK Bromley & Greenwich is looking to appoint 2 Care Navigators to work with health colleagues at the new acute frailty unit at the Princess Royal University Hospital.  

The Care Navigators will work alongside health  professionals and are pivotal in capturing the patient’s non-medical support needs and then navigating them to the right support, at the right place and time.  You will be working with other disciplines on the Bromley Well project, ensuring that the citizens of Bromley have access to primary and secondary support services. In addition, the Care Navigator also signposts and/or refers to third sector services of benefit and provides information and encouragement to support patients be empowered to make informed decisions about their health and wellbeing.

We are seeking applications from individuals with experience of delivering services in the third sector, health or social care for with people with complex needs. You will need to demonstrate a compassion and empathy for people in a professional manner, have excellent communication, organisational and IT skills and experience of building relationships across the sectors.

In return, the successful candidates will be provided with comprehensive training to fulfil the role.
