Age UK Bromley & Greenwich is looking to appoint a Care Navigator to join a dynamic team committed to supporting the provision of high quality health care to people within the London Borough of Bromley.
The integrated Care Networks (ICN) have been co-developed with key stakeholders for providing more integrated care services, initially for people with complexed needs or long term conditions who are at high risk of future admission to hospital, helping them stay healthier at home.
The Care Navigator, works alongside multidisciplinary team professionals and is pivotal in capturing the patient’s non-medical support needs and then navigating them to the right support, at the right place and time.. The Care Navigator also signposts and/or refers to third sector services of benefit and provides information and encouragement to support patients be empowered to make informed decisions about their health and wellbeing.
We are seeking applications from individuals with experience of delivering services in the third sector, health or social care for with people with complex needs. You will need to demonstrate a compassion and empathy for people in a professional manner, have excellent communication, organisational and IT skills and experience of building relationships across the sectors.
In return, the successful candidates will be provided with comprehensive training to fulfil the role.
