Personal Independence Coordinator and Reablement Link Service Manager

Salary £35,000 FTE fixed term for 9 months Maternity Cover (with an option to extend to 12 months)

35 hrs per week

This is an exciting opportunity to lead pioneering programmes which are at the heart of Croydon’s New Model of Care, driving preventative, holistic, people-centred care. The programmes are funded by One Croydon Alliance, a partnership between Croydon Council, Croydon CCG, SLAM, GP collaborative and Age UK Croydon. These programmes are at the front-line of innovation, integrating the statutory and voluntary sector to support older adults in their community.

We are looking to recruit an experienced Manger to lead the team of Personal Independence Coordinators (PICs) and Reablement Link Workers. The PICs and Reablement Link Workers work alongside health and care professionals, adopting a multidisciplinary approach to working with older people who have long term health and social care needs. Most importantly, they ensure that care planning is influenced by the wishes identified by individuals, so that independence can be either improved or maintained. 

The Service Manager will be responsible for the operational and strategic management of this project, ensuring the delivery of the programme on time and within budget.  Direct line management of 4 Team Leaders with a service staff team of 23 will need strong team building and personnel management skills.
