Job description

Job purpose

City Connections is a programme of services which links people to health and wellbeing services within the Square Mile. They may live or work in the City, are over 18 or a young carer, and would like help to understand what services would suit their needs and how to access them.

The City Wellbeing Co-ordinator will work within a small, focused and highly creative team which includes a Care Navigator and Community Engagement Officer in order to deliver a person centred, tailored service.  The service has cross over with a number of other key Age UK East London services, such as the Older People’s Reference Group and there will be ongoing collaboration with these. There will also be opportunities to participate and contribute to senior level reviews and presentations at the highest levels of the Corporation of London’s governance framework.

The service has evolved significantly since its inception nearly 3 years ago, not least in response to the pandemic. User involvement has been central to this process and ongoing engagement and feedback on the service offer from our customers is a key feature of the role.

You will be joining at an exciting time as we continue to develop the service offer and emerge from the pandemic. 

Key tasks

- Clients 

  • Being the initial point of contact for referrals and conducting client focused health and wellbeing assessments; via home visits/office meeting/estate office/telephone or remotely.
  • Joint home visits with colleagues/partner agencies where required/applicable.
  • Signposting and referring to internal and external services, with follow up communication with clients and list of action plans
  • Completing all detailed casework with clients, follow up visits/meetings, reviews where appropriate, in a timely manner and record updates onto database.
  • Liaising and working with ASC Duty and other local organisations and agencies to provide the best support for City Connections clients, with follow ups and update check as required.
  • Assisting with supporting the Carer’s sessions and forums and any other City activity sessions as needed (in-person/virtual).

- Evaluation and monitoring 

  • Accurate and timely recording of new client data, sessions and reviews held onto the Charity Log database
  • Compiling and completing all necessary data and stats for quarterly reports from databases – using set outcome measurements, including service user surveys, case studies and break down of health and wellbeing referrals

- Marketing & Events 

  • Attending and representing at City events, conferences, forums and training sessions
  • Attending at relative provider meetings, forums; networking to establish new opportunities for the City project if applicable
  • Identifying new promotional sites and opportunities within the City to enhance promotion of the service and to work with colleagues to capitalise and monitor results
  • To build on, and maintain, relationships and partnerships with local stakeholders including local community groups, local authority departments and health providers
  • Writing, collating and distribution to clients of the City Connections Newsletter on a monthly basis.

Person specification

Experience

  • Experience of delivering person centred services with a focus on health and wellbeing that meet defined outcomes and outputs
  • Proven experience of monitoring and evaluation
  • Proven experience of confidentiality policy and practice
  • Proven experience of equality and diversity, GDPR in practice
  • Proven experience of using a CMS database system 

Knowledges and understanding

Knowledge of services available to City residents/workers and their carers (e.g. through health service, social services, voluntary sector) and the issues and barriers for them to access them for different client groups.

Additional requriements

  • Some marketing/publicity experience desirable
  • This post is subject to enhanced check through the disclosure and barring service (DBS);
  • This post requires travel across the City of London; office base will be Bethnal Green, East London
  • The post holder will work with standard office server based computer systems and the Charity Log Management
