JOB DESCRIPTION

JOB TITLE:                  Community Supporter

HOURS:                  37½  hours per week (office hours 9am to 5pm with                     ½ hour unpaid lunch break)

SALARY:                 £22,600 p.a

RESPONSIBLE TO:             Head of Service, Information, Advice and Advocacy

SERVICE SUMMARY:

Age UK Essex has accessed grant funding to enable the provision of high quality. Professional Information and Advice to vulnerable people, their families and carers within Writtle, and the surrounding areas and villages.

ROLE SUMMARY:

  • The Community Supporter will be employed by Age UK Essex.

  • Although this role is community based, the post will be embedded within and fully part of the Information and Advice service which is based at Widford Hall in Chelmsford.

  • The Community Supporter will report to the Head of Service for Information, Advice and Advocacy, who retains responsibility for the overall delivery of the service.

RESPONSIBILITIES:

  • To develop links with “community leaders and influencers”
  • To foster and develop referral pathways from the community to the service

  • To deal with enquiries from older people, their families, carers, friends and other interested parties, principally in community settings or in the client’s own home but also by telephone, post or email.

  • To provide appropriate and timely advice and information that will enable people to maintain independence.

  • To help people retain or access suitable local accommodation

  • To increase levels of income through access to benefits that they are eligible for.

  • To increase wellbeing by empowering people to make positive changes.

  • To strengthen local communities and assist them to identify their own needs.

  • To join local people, services and resources together to strengthen resilience.

  • To deliver advice and information directly into communities through clinics and events.

  • To maintain accurate and timely records and make case records and reports as appropriate.

  • To evidence the outcomes and benefits of the work undertaken through monthly case studies

  • To evidence the work of the Service and to gather, update and organise relevant information.

  • To attend training events in order to remain up to date on developments on the issues.

  • To represent Age UK Essex professionally at all times.

  • Such other duties as may from time to time be reasonably required by the Charity. 

COORDINATION AND INTEGRATION:

  • Develop relationships with key stakeholders at local level to ensure knowledge of local services is up to date

  • Identify when there is a need for urgent action or for a step-up in care and alert the relevant professional(s) and Age UK staff and our services.

RECORD KEEPING, PROJECT EVALUATION AND RISK MANAGEMENT:

  • Keep accurate and up-to-date records of client contact. 

  • Record and collate information, including case studies, to demonstrate the impact of the service

  • Contribute to development and coordination by attending meetings as requested. 

  • Identify opportunities and gaps in services.

  • Assessment and management of risk to ensure the safety of self and security of clients
