We deliver a simple route to providing adults (16 yrs+) with support to overcome issues that are getting in the way of an improvement in their health & wellbeing, e.g, helping with debt, housing issues, emotional or bereavement support, social isolation and much more.
We are looking to recruit an experienced client-focused leader who is passionate about delivering services that make a difference to people’s lives. You will lead a small team and have responsibility of maintaining a high level of service delivery with impressive outcomes. You will be highly organised and able to make decisions and use your initiative.
The purpose of the Carers & Community Manager job is to manage and deliver the day to day operations across the Carers and Community support services. Over the last two years we have widened our service focus to support people of all ages achieve and maintain Wellbeing –‘Wellbeing’ is a broad concept and is described as how people feel and how they function, both on a personal and a social level, and how they evaluate their lives as a whole.
With a strong focus on navigation and social prescribing, our teams are closely integrated with statutory and community service provision – and providers – across the borough. Now we want to go further: to reach more people, to help them move to independence faster and more sustainably, to connect with wider/more community activities and services, to be even more personalised in our approach, achieving better outcomes whilst also improving quality and user experience.
The specific focus of the role is on managing service delivery to ensure that service activity and performance is effective and successful. The role will also involve monitoring service processes, systems, and workforce capacity to enable us to identify areas for change and improvement.
Manager: Carers & Community
Services within this area include information and advice/guided-choices, carers support and assessment, social activities coordination and Helpline/ First Point-of- Contact/engagement hub operations.
Service provision consists of population-based information provision (including to family and neighbour carers), Helpline enquiry handling and one-to-one advice and guided-choices appointments, social activity promotion and events/campaigns, proactive Wellbeing 360 checks. Service operations involve reaching larger numbers of people, spotting, and acting on signals of change in wellbeing – positive and negative – acting as appropriate and relevant. Service delivery relies on wide and regular engagement and connections with local communities incorporating a range of technologies.
