Job description

Age UK Islington specialises in Wellbeing services provision. Our range of services is available to all adults and we have an exciting opportunity for two enthusiastic Social Prescribing Link Workers to join our dynamic team. Social prescribing is at the forefront of the NHS long term plan to help patients/clients find much needed support from community and voluntary sector organisations.

This role is well suited to individuals with experience of directly working one-to-one with patients/clients, families and carers, to plan and help people resolve practical issues such as Housing, Finances Employment alongside wider wellbeing support, such as keeping active and connected. Based within Islington, you will be working closely with Primary Care Networks, GPs, Health & Social Care professionals, together with other Age UK Islington wellbeing services.

You will have responsibility for managing a caseload, triaging referrals, case work management, connecting people to specialist services or community services to sustain independent living, health and wellbeing.

Your role involves working with people who will be experiencing complex issues, so you will need good listening and communication skills and be able to build effective partnerships with a wide range of organisations and community groups. 

You’ll need excellent IT and organisational skills, be adept at seeking out the information for relevant community services and have a tenacity to help people achieve sustainable improvements in their wellbeing.
