This is a part-time role (25 hrs a week)

The Lambeth Advance Care Project (LACP) is a group of local community organisations (Age UK Lambeth, Carers Hub Lambeth, St Christophers Hospice, Stockwell Partnership, Ask Erwin, Healthwatch Lambeth, Lambeth Learning Disability Forum) supported by a national charity, Compassion in Dying. We raise awareness and encourage people in Lambeth to plan and record future treatment and care wishes. The LACP offers specialist support for local people to create a Lasting Power Of Attorney (Health and Welfare) and support with Advance Care Planning tools: Advance Statements and Advance Decision to Refuse Treatment. 

Advance Care Planning tools are the three main ways we can record our wishes for treatment and care if we cannot make a decision for ourselves or tell people what we want

What you will be doing

Plan, promote and deliver advance care planning events and awareness sessions in Lambeth. Be creative in methods to attract people into sessions e.g. wills & forward planning workshops. And also;

  1. Support people to complete Advance Care Plans (ACP) and where possible link people to our ACP volunteers.

  2.  Provide person-centred support allowing individuals the time and space to discuss “what matters most to me” is where they are most comfortable. In their home, our office or just a place of their choice.  

  3. Support people to complete Advance Care Plans and also where possible to link people to our ACP support volunteers to help them complete Advance Care plans

  4. Liaise with referrers to confirm support and outcomes. Referrals are received from a variety of agencies such as GPs, Living Well Network, Adult Social Care and voluntary groups.

  5.  Work with Age UK Lambeth’s  MYcommunity, MySocial and other services and organisations across the borough to promote the events and to encourage referrals. (For more information, please see Job description attached)

  1. A major part of the role will be attending Community events. How do you think you could attract people to stop at your info stall and speak to you? 

  2. You may be asked to give a presentation either at an event in person or to a virtual group. Would you be comfortable doing this and have you given a presentation before?

  3. Many people find it uncomfortable speaking about dying and planning for it. How would you approach this topic when speaking to Individuals.

  4. How would you go about building community links in the borough?
