Job description

Age UK Lewisham and Southwark (AUKLS) aims to empower and enable older people to lead fulfilled lives.

We do this by:

  • Providing services and support to address poverty and isolation
  • Connecting older people with their communities
  • Promoting health and wellbeing
  • Working positively with partners

We are looking for a new Social Prescribing GP Navigator to help older people access healthcare services, their local community, and activities. This will be through face-to-face appointments in the office, GP surgeries, home visits, telephone calls and letters. We need confident self-starters, able to work in busy enviornments while building good relationships with a variety of professionals.

This will be rewarding role giving you the opportunity to make a real difference. Not only will you directly help the people we support to improve their lives, indirectly you will be doing vital work to ease the pressure on NHS.  Furthermore, there will be opprtunities to attend training, events, forums and meetings to broaden your understanding of Health and Social Care and build experience working in the voluntary sector. You will also be working closely with our other social prescribing projects in the borough strengthening our offer of support in Southwark.

 

This post is for six months (end of September 2021) with likelihood of extension subject to funding.
