We have multiple posts available and have hours from 21 through to 35 hours per week. We offer flexible working and, where possible, will fit your hours to your personal needs. We do require a minumum of three working days a week. You will be based from home working within a "patch" of Oxfordshire. We are looking for people to work predominantly across South Oxfordshire, Oxford City, Vale of White Horse and the Cherwell area.
Working within local communities across Oxfordshire you will be joining our successful Community Information Network Team as we work to further improve access to information, support, activities and opportunities for all those with a health, care or support need. You will be working with individuals, families and groups both in the community and in their own homes. In addition you will be working closely with local GP surgeries to provide the new social prescribing element into the support they offer to their patients. You will also have the opportunity to join our hospital discharge support team working across various settings including the John Radcliffe and Horton Hospitals and community hospitals. We work with local communities to further build upon and support what is being offered as well as working with individuals and their families to connect them with the support and opportunities that exist. We are passionate about supporting and empowering people so that they can maintain their independence, health, wellbeing and ‘live life to the full’.
