Care Navigator/Social Prescriber Home First
Hours: 28 - 35 hours per week

We are looking for a Care Navigator to join our Care Navigation team supporting people with multiple long-term conditions and other specified groups to self-manage ensuring they have maximum support at home, enabling choices and access to services in the community to achieve the best possible quality of life and independence.
The Care Navigator will provide time-limited guidance to people with long term conditions and to their carers as well as other specified groups. Key duties include establishing a support plan with the patient and carer; signposting the patient and carer to services available in the community.
Successful applicants will have experience of working with older people or people with multiple long term conditions in health, social care or advice and information, in a paid or voluntary capacity. They will need to be excellent communicators and able to work effectively both one to one and as part of a team as well as supporting and motivating volunteers. Good computer skills and accurate record keeping are also required. An enhanced DBS Disclosure is required for this post.
