Job description

An exciting addition to our team.  Empowering people to take control of their health and wellbeing.  Working with the local GP networks and taking a holistic approach you will support people to connect back into their community through practical and emotional support.  

You will be an innovator, who is self motivated, and who has knowledge and experience in the social care or health fields.

There are three posts available - 1 full time (35hrs per week) and 2 part time (21 hours per week).  The part time posts have an option to increase hours should the demand require it.  The full time post is initially for 12 months and the part time posts intially 6 months. If successful there is potential for the posts continuation beyond initial term.
