Job description

We are searching for three kind, friendly and patient people to join our brand new Outreach Service. Our pilot Outreach Service will deliver flexible support to older people, adults with disabilities, and adults with dementia in their homes. The aim is to develop individual resilience, increase mental and physical wellbeing, and provide respite support for carers. Support  will mainly focus on the provision of practical and engaging activities in client homes and support to develop and maintain independence. Outreach could be provided in a combination of home-based, AUKW office and community settings. Clients will be referred directly into the Service by Wandsworth Council’s frontline social care teams.
