Social Prescriber

Salary:                           £26,265 - £28,366 per annum + 6% pension

Hours:                            36 hours per week

Based at:                       Various GP Surgeries as assigned

Contract:                      Two years (possible extension)

The Bridge Renewal Trust, in partnership with the NHS, are delivering a social prescribing service in Haringey.   

As the Social Prescriber, you will provide follow on support to service users from the assessment and care plan, or from interactions with other professionals such as the GP.  You will provide holistic assessments and co-design social prescription to improve health and well-being outcomes of individuals.

Are you a passionate and dynamic individual with excellent organisational and interpersonal skills; and a proven track record of working in a multi-disciplinary team, working closely with GP practices and other healthcare professionals, the general public and voluntary sector?

We are currently recruiting a Social Prescriber who will focus on helping patients to make full use of local community and voluntary sector services available locally, as well as those provided by health and social care providers.  The key requirements for this post are experience of working with clients providing person-centred care and being familiar with voluntary and community services to respond holistically to patients’ needs.
