Greenwich MindCare Dementia Information and Support Service
Bromley, Lewisham & Greenwich Mind (BLG Mind) are commissioned by the Royal Borough of Greenwich to deliver the dementia advice and information support service. Greenwich MindCare supports people with a dementia diagnosis and their carers and works in close partnership with Oxleas Memory Clinic. The dementia advisors are situated alongside the Memory Service at Memorial Hospital thus ensuring a seamless route from diagnosis to accessing advice, information and support.
Greenwich MindCare service provision includes:
- Personalised advice and information
- Individual person-centred support planning
- Help to access other support, services and community resources
- Carer-specific workshops
- Follow up support and contact
Dementia Advisors provide advice and person centred individual support, including signposting and bridge building to help people access community resources. We are seeking applicants with experience of working in health or social care, including in community based services, who possess good communication, assessment and support planning skills.
