We’re Mind, the mental health charity. We’re here to make sure anyone with a mental health problem has somewhere to turn for advice and support. We provide a range of support services across Buckinghamshire including day services, education and training, counselling, services to prevent people from becoming unwell, befriending, peer support groups, children and young people’s services and employment support. We have an active Service User Council who ensure that we provide a voice for people with mental health problems in the county. With one in four people experiencing a mental health problem at some point in their lives, people need Mind more than ever.

Post Title: Information and Options Worker, North Bucks

Reports to: Buckinghamshire Mind Outreach Services Team Lead/Recovery and Wellbeing Manager & CMHT Clinical Lead/Band 6 Supervisor

Salary: £22,500 per annum

Hours of Work: Full Time 37 hrs/week: Monday-Thursday 9am-5pm, Friday 9-4.30pm

Contract: Permanent

Work Location Based in the Whiteleaf Centre, Aylesbury, with travel across North Bucks and working from home if Covid restrictions require.

PURPOSE OF THE JOB

The Information and Options Worker will help to raise patient awareness of alternative mental health services appropriate to personal need and circumstances as well as encourage and support service users to use these services to improve longer term mental health and well-being.

Working closely with the Community Mental Health Team the postholder will take referrals (post assessment or during treatment) and signpost patients aged 18-65 to appropriate community support services – both within Bucks Mind and with other service providers.

The post holder will work closely with service users to understand their recovery goals and provide ‘handholding’ support to enable service users to attend the groups/activities in the community. A high level of work will take place in community as well as from within the Whiteleaf Centre across varied short-term periods of work (often no more than 3 months with each service user).

The post holder will keep monitoring and impact data, provide regular stats and collate printed publicity material and information to supply to service users when required. Close liaison with Buckinghamshire Mind’s teams will also be required to ensure referrals into Buckinghamshire Mind services are well supported and that information and publications are up to date and available.

SUPERVISION, TRAINING AND SUPPORT

  • Supervision is provided on a monthly basis by both Oxford Health and Bucks Mind.
  • Buckinghamshire Mind has a system of annual appraisals.
  • Buckinghamshire Mind is committed to training and allocates training according to a clearly defined annual training needs analysis.

RESPONSIBILITIES

Raising Patient Awareness

  • To take referrals from a number of sources within Oxford Health and dependent upon decisions taken during patient assessment and/or treatment have a 1:1 consultation with patients at the Whiteleaf Centre, patient’s homes and in community locations, and to provide details of the range of (support) options available in the community, signposting, referring or supporting as required.
  • Provision of an individual face to face ‘options sessions’ for people at assessment stage who do not meet treatment threshold or are discharged from the CMHT treatment team. This will be done remotely if required by social distancing measures.
  • Providing a digital option for the service, when and if required by Government guidelines on social distancing. This includes providing weekly activity packs/care packages sent via post, Zoom meetings/social activities, Facebook engagement and phone calls.
  • Where needed take patients to the community groups/activities/services to promote their ability to attend the service regularly and independently.
  • Liaise with Care Coordinators to agree on patient chosen activity and discuss future community-based support options as required.
  • Ensure that the named Care Coordinator (or other referral source) is made aware of the recommendations so that they can be followed up.
  • Attend the OH Service User Forum and Carers Forum and other meetings where patients will be attending to promote awareness.

Monitoring

  • Monitor patient progress by liaising with CMHT colleagues and/or accessing electronic patient records.
  • Reporting of service evaluation and outcome measures such as caseload, referral source, referrals, signposting recommendations, needs, progress measures, impact.
  • Attend Multidisciplinary, Business meetings and any appropriate forum or team meeting, such as reflective practice, requested by manager to discuss and share information around potential patients.

Supporting and developing the Bucks Mind Information Service

  • To support the development and provide updated information for the Buckinghamshire Mind Guide database and website.
  • To liaise with outside organisations, individuals and the community at large to ensure the information we hold and distribute is correct, up to date and appropriate.
  • To work with Bucks Mind colleagues to ensure that the online Bucks Mind Guide is well promoted and utilised across partner and community organisations.
  • Monitor gaps in services and seek to mobilise additional provision to meet the needs of service users.

Interpersonal skills

  • To communicate appropriately and effectively with service users and professionals across Bucks Mind, Oxford Health and the wider community.
  • Demonstrate effective teamwork with the Information and Options Worker based in the South CMHT to ensure quality of service provision, attending weekly peer support where possible.
  • To recognise, challenge and be responsive to stigma and discrimination of all kinds.
  • To represent the organisation in a professional and appropriate manner at all times.
  • To work creatively, looking at new possibilities, bringing new ideas to the team and respond appropriately to the needs and views of both current and potential service users.
