The Role
Our Practitioner Led Assessment Service is the entry point for our beneficiaries seeking support from the Charity. Our Assessment Practitioners work in partnership with our beneficiaries to identify goals, individual needs and appropriate solutions.

The team provide advice, information and support and work collaboratively with the multi-professional team to ensure our beneficiaries are offered the right support at the right time.

Reporting into the Beneficiary Experience and Continuity Lead you will be responsible for day-to-day management of the practitioner led assessment service, holding a small case load and reviewing and resolving complex cases with the team. 

You will work collaboratively with key stakeholders across health improvement and rehabilitation services, to ensure services are integrated, person centred, outcome focused and based on an ethos of recovery.

About You
An excellent communicator, with a can-do-attitude, you will be a registered health or social care practitioner with a broad skill and knowledge base and will be experienced in managing a multi-professional team.

To be successful, you will:

• Be a registered health or social care practitioner, with membership of a relevant professional body
• Have experience of managing a multi-professional team within a health or social care setting
• Have experience of conducting telephone assessments (health and wellbeing), service user risk assessments and delivering advice and information to improve health and wellbeing
• Be able to communicate effectively with people at all levels, both verbally and in writing, together with sound judgment skills
• Have the ability to travel (within the UK) on occasion

Part 1: Job Profile

Main purpose of job

The post-holder will be part of a multi-professional team providing holistic care and support

for children with life-limiting condition and their families. The post-holder will work in

collaboration with the family and other services providing care and support for the family.

The role includes active involvement in the education, training and professional development

of other team members including volunteers.

The Shooting Star Children’s Hospices care service is provided 24 hours a day, 365 days a

year. The post holder will be required to work a variety of shifts across the 24 hours.

Band 6 roles will also participate in the on call rota.

Part 2: Main duties and key responsibilities

Main duties and key responsibilities

• Act in accordance with Shooting Star Children’s Hospices policies, guidelines and

standards. To keep up to date with national policies and guidance.

• Adhere to the NMC Code of Professional Conduct at all times.

• Participate in the assessment, care planning and care management of each child and

family receiving care from Shooting Star Children’s Hospices.

• Develop and maintain clinical skills and knowledge necessary to provide holistic,

Evidence-based nursing care. This includes completion of the Shooting Star Children’s

Hospices care competencies

• Liaise with clinical, non-clinical and facilities staff in order to maintain a safe, friendly and

welcoming environment for the children/ young people, families, visitors and staff

• Ensure cleanliness and infection control standards are maintained within the clinical area

taking appropriate action where these standards are not maintained and escalate to

Lead Nurses/ Head of Hospice Services where necessary

• Act as an advocate for the child / young person and family ensuring the provision of

appropriate information and support services

• Ensure effective communication between all members of the multidisciplinary team,

parents, relatives, visitors and external agencies

• Participate as a team member in the other activities that complete the care cycle, i.e.

Hospice Services, ensuring that all Shooting Star Children’s Hospices property and

equipment is well looked after and maintained.

• Inform the relevant team member of any ordering required to facilitate good stock control

Post Title Progression Worker 

Responsible to Director of Services (subject to review) 

Purpose of Post To support users of services through an education, training, and employability programme integrated with SCT’s wider services and enterprise offer, as well as partner opportunities in the wider community. To oversee and coordinate access to SCT’s Training and Development tutor-led activities including art, creative writing, digital skills, gardening, literacy, woodwork, and peer-led ‘Choices’ groups, and to monitor people’s progress over time.

Spitalfields Crypt Trust (SCT) is an East London charity embedded in the local community, providing practical support and training to people affected by homelessness and addictions so they can rebuild their lives. We have a Recovery Hub in Shoreditch, where we run an abstinence-based residential recovery service (‘Acorn House’), and a Training and Development service, including tutor-led art, creative writing, digital skills, gardening, literacy, and woodwork. In addition, we run Housing First and Supported Housing services, both in East London. We are dedicated to supporting those who access our Training and Development service to explore further Education, Training, and Employment (ETE) opportunities as part of SCT’s Progressions Programme, both within and outside of SCT. This includes those in early recovery who are resident in our abstinent-based recovery service (‘Acorn House’), as well as those in our move-on house and the wider community who may be more established in their recovery. SCT’s Progression Worker supports those accessing our services to explore ETE opportunities, offering tailored, individually specific support to help them reach their full potential. SCT’s Progression Programme is centred on empowering people in recovery to cultivate confidence and realise their potential. 

SCT Progressions Programme 

1. Identifying SCT residents and students ready to pursue ETE opportunities: 

• Collaborate with SCT tutors and managers, particularly the Therapeutic Programme Manager and Supported Housing Manager, to identify those who may benefit from the support on offer. 

• Cultivate strong relationships with SCT’s social enterprises and shops to establish clear pathways for volunteering and trainee opportunities for those who access our services. 

• Conduct assessments, develop and implement individualised Progression Plans, and evaluate progress according to each person’s needs, strengths and aspirations, involving key internal and external partners as appropriate. 

1. Providing information, advice, and guidance regarding ETE opportunities: 

• Offer information, initial advice, and guidance on employment, voluntary work, and further education opportunities.

About the role 

You’ll bring expert knowledge of Parkinson’s pathophysiology, pharmacology and therapeutics development to all our VB activities. These include primarily the sourcing and evaluation of new project proposals, and then the funding and management of the most promising ones. You will liaise extensively with the Parkinson’s community and external partners, and maintain up to date knowledge in the field. 

With your years of preclinical disease-area expertise you will work alongside your colleagues with backgrounds in medicinal chemistry and biological aspects of drug discovery, clinical research and biotech finance.

What you’ll do:

• Provide expert knowledge of Parkinson’s pathophysiology, in vitro and in vivo models of aspects of Parkinson’s and neurodegeneration, to all our activities.

• Solicit external proposals for new projects and develop internal project proposals, and bring new initiatives and working practices to the Virtual Biotech, as appropriate

• Critically review the target validation and preclinical pharmacology aspects of internal and external project proposals

• Participate in joint steering committees and other advisory meetings, providing expert scientific and strategic insight

• Interact with people with Parkinson’s and involve them in projects in order to increase the opportunities to deliver new drugs that meet unmet medical needs

What you’ll bring:

• Widely respected expertise in the study of Parkinson’s pathophysiology and preclinical pharmacology with at least 8 years post-doctorate experience in the field, and a track record of proposing new projects and securing the required funding

• Doctoral degree in a relevant discipline (Ph.D., M.D., D.Phil.)

• Familiarity with small molecule drug discovery principles and practices including use of Parkinson’s disease models

• Excellent communication skills, with the ability to present complex information in suitable formats, including project proposals/reports, for both scientific and lay audiences

• Significant experience in project management and leadership skills from initiation to completion/review, with the ability to lead cross-functional teams

A current and key area of our focus is Employment across London and beyond, we work in close collaboration with a range of stakeholders and commercial partners to deliver supported internships as a pathway to paid employment and a career.

Selected interns undergo a full academic year-long placement in a commercial work setting, gaining valuable work experience and essential employment skills. Throughout the internship, tailored job support and pastoral oversight is provided by Job Coaches and Managers working on-site, in our partnership settings. Generate has been building stakeholder partnerships at the forefront of supported employment internships for young people with learning disabilities since 2018. As Partnerships Manager, your collaboration with the Employment Manager will lead on the delivery of supported internships. Responsibilities include: managing multi-site projects and a growing team, recruitment and training initiatives, securing funding and fostering key partnerships with local authorities and colleges. Your contribution will be to manage and lead the sustainable growth of our supported internship offerings in alignment with our organisational objectives.

We are seeking a highly skilled business professional to cultivate and nurture these critical relationships. You will have experience of multi-site project management, data analysis and reporting, plus exceptional people management skills. Experience in the employment sector/internships/graduate recruitment would be of interest. A degree in business/marketing/communications or similar preferred.
