Job description

Are you fully committed to person-centred services? Will you always “go the extra mile” to make sure people find the best possible solution to their needs? We are looking for a part time (18.5 hours per week) community Navigator.

Seven voluntary organisations have formed a partnership to develop a Herts-wide Integrated Hospital Discharge and Community Navigation Service (IHDCNS), with teams based locally that will understand their local populations, and support people to find and access community-based support. We have two exciting opportunities to join the team in Hertsmere currently funded until March 2020 but with potential for extension. This is part of exiting new developments nationally in social prescribing and link workers and an area where new opportunities are emerging for staff and services.

The successful applicant will provide a service that puts people first and empowers them to make appropriate decisions about their care and access support and services in their own community. Reporting to the Senior Navigator you will hold your own caseload and support others in the team as required. You will build a strong understanding of voluntary and community support across Hertsmere and have the ability to creatively problem solve bringing agencies together to design imaginative solutions for individuals.  You will have the ability to work alone and as part of a team, be self-motivated and able to inspire others to work with you.
