Senior Navigator – Hertsmere

Salary £26,500 per annum (37 hours per week)

Seven voluntary organisations have formed a partnership to develop a Herts-wide Integrated Hospital Discharge and Community Navigation Service (IHDCNS), with teams based locally that will understand their local populations, and support people to find and access community-based support. We have an exciting opportunity to join the team in Hertsmere, which is currently funded until March 2020 but with potential for extension. This is part of exciting developments nationally in social prescribing and link workers and an area where new opportunities are emerging for staff and services.

We are looking for a Senior Navigator to lead a team of Community Navigators to provide a service that puts people first, empowers people to take appropriate decisions about their care, and to access support and services within their own community. The post holder will be responsible for

  • Allocating cases appropriately across the team, taking into account their experience and existing workload

  • Motivating and supervising staff to manage their caseload and ensure quality of the services is maintained.

  • Maintain own caseload balancing this with the management of the team.

  • Build and maintain strong links in the local community with GP’s, social care, voluntary and community groups and other relevant organisations.

Reporting to the Service Area Manager you will be a self-motivated individual who can inspire others. With an ability to analyse data in order to determine workloads and meet targets you will be confident to talk about and promote the service in a range of settings.
