Team Leader – Hospital and Community Navigation Service - Hertsmere

Salary £28,000 per annum (37 hours per week)

Carers in Hertfordshire has partnered with seven local voluntary sector organisations in Hertfordshire to deliver the integrated hospital and community navigation service (HCNS).

We support local people to find non-clinical solutions to their health and wellbeing needs and help to integrate with community services.   The aims of this  service are to help improve health and wellbeing, reduce hospital (re)admissions, reduce GP visits, reduce reliance on home care providers, and reduce social isolation.

As the Team Leader you will manage a team of hospital and community navigators and will be responsible for resolving safeguarding and urgent issues. This role will be based at our offices in Borehamwood, but will include remote working and require travel across Hertfordshire.

Strong leadership, communication and team working skills are essential for this role, in addition to an understanding of the needs of frail and vulnerable people. You will also need to have an in-depth knowledge of safeguarding and health and social care.
