Job description

Do you have experience of managing service delivery in a community, health or social care setting? Are you passionate about making a difference to the lives of unpaid friend and family Carers?

We are a successful charity provider working across the county with a critical service contract from West Sussex County Council to support unpaid friend and family Carers. We have a job share vacancy for a Community Team Leader

Key responsibilities include;

  • Lead a county-wide team of Carer Wellbeing Support Workers to provide best value, high quality advice, assessment, information, support and advocacy for family and friend carers.
  • Take responsibility for operation and development of programmes within specified areas
  • Ensure Carers are offered a choice of personalised and time-focused support to best meet their needs/preference including 1:1 casework, groups, telephone/web-based support.
  • Ensure strong integration with local health, social care and wellbeing support providers so that the team form and maintain effective working partnerships and maximise wellbeing support opportunities for Carers

  • Monitor, evaluate and report on activity and quality for contract compliance and internal management reporting.
  • Ensure our community services are shaped by carer views and opinion
  • Responsible for the recruitment, training and performance management of staff in the community team
  • Participate in the duty manager rota
  • Ensure all safeguarding concerns are recognised and reported to the safeguarding lead
  • Work closely with the Fundraising team to support growth and development of the CSWS community Service.

This is a 50:50 job share working 19 hours per week over 3 days, days to be agreed. You will share the management of activities, projects and staff with the other Community Team Leader.

You will have experience of managing service delivery in a community, health and social care, primary care or hospital services setting. Also a good understanding of Carers’ issues and needs, good communication and organisational skills together with an empathetic caring approach. Based from in our Worthing office, the role requires regular travel across the county.
