We are looking for someone who can provide daytime, some evenings and some weekend, visiting support to young people living in their own accommodation in the community. You will provide a structured and responsive support to the tenancy sustainment needs of young people living in their own accommodation. You will also provide resettlement support, information and guidance for young people identified as ready for move on in preparation for the transition from Ty Bronna to living in the community. You should have good relationship skills to develop and sustain relationships with young people, partners, stakeholders and new and existing housing providers.
