Hours:part time (up to 22.2 hours per week) as part of a job share.

Location: Working from home (due to the pandemic)

Role linked to the Banbury office. Must be able to travel across North Oxfordshire (when safe to do so)

Citizens Advice North Oxfordshire and South Northants is recruiting for a Community Navigator for our Social Prescribing project, to help clients who would like to become more active and find social connections.

Do you have experience in coaching and empowering people? Do you have at least a year's experience in either health and wellbeing or the voluntary sector?

Social Prescribing is a mechanism for linking primary care patients with non-medical sources of support within their community. It attempts to address the social causes of ill health and aims to support patients, who are experiencing loneliness, isolation or inactivity, which is detrimental to their health and wellbeing. GP’s and the health teams, as well as community health services will make referrals. We will also invite self-referrals.

As a Community Navigator, you will be a vital and visible community resource, responsible for patient engagement and support and for developing effective working relationships with health care professionals and third sector providers.  

We are looking to recruit a caring, dedicated, reliable and person-focused individual who enjoys working with a wide range of people and supporting a team of volunteers to deliver exceptional service.

You will need to have good written and verbal communication skills and strong organisational and time management skills. You will have a commitment to empowering patients using coaching techniques. You will also engage with volunteers to support your work.

As this is an evolving role, we are looking for highly motivated and proactive applicants with a flexible attitude who will be committed to providing patients and carers with high quality support.
