Job description

We are looking for Social Prescribing Link Workers to join our friendly, dynamic and progressive organisation, working across Healthier West Wirral Primary Care Network (PCN) and with the
voluntary, community and faith sector.

Social Prescribing Link Workers form part of the multidisciplinary teams in Primary Care Networks and are uniquely placed to work closely with GPs, local authorities, health and care professionals and voluntary sector partners to coordinate support for people who are self-isolating during this Covid-19 outbreak and beyond. The role

Social Prescribing empowers people to take control of their health and wellbeing through referral to Link Workers who focus on ‘what matters to me’; take a holistic approach to an individual's health and wellbeing, give time and connect people to community groups and statutory services for practical and emotional support.

We are looking for enthusiastic and motivated Link Workers to join the existing team, extending the Social Prescribing service to deliver 1:1 personalised care to a wider patient population. The successful candidate will work within one of the Primary Care Networks in Wirral, supporting General Practice and work with people who have long-term conditions (including support for mental health), people who are lonely or isolated, those who access primary and secondary care service frequently or who have complex social needs which affect their wellbeing.

The Candidate
The successful candidate will be able to demonstrate the ability to be a good listener, have time for people and be committed to supporting local communities to care for each other and reduce health inequalities. You should have experience of working positively with people facing complex social and emotional challenges in either a paid or voluntary capacity; inspiring trust, building confidence and motivating. You will have great interpersonal skills; be empathetic, resilient and be able to maintain effective working relationships with partners across the sectors.
