Job description

We are looking for a self-motivated and community-minded individual to provide a social prescribing service to people over the age of 18 living in the London Borough of Lewisham.

Community Connections Lewisham is a preventative social prescribing project, delivered by Age UK Lewisham and Southwark, aiming to improve the health and wellbeing of vulnerable adults across the borough of Lewisham. We achieve this through working with both individuals who want connecting to services, and the community groups they access. We support people to find:

  • Local community groups and access to social opportunities
  • Help with staying active
  • Befriending and practical assistance (food shopping, odd jobs, etc.)
  • New opportunities for employment, training and learning
  • Volunteering opportunities and opportunities for participation in local projects

Community Facilitators take a holistic person-centred approach, connecting people to community groups and statutory services for practical and emotional support. They support their clients to tackle the wider determinants of health, such a debt, poor housing and physical inactivity. Community Facilitators strengthen community resilience and personal resilience, thus reducing health inequalities in the borough.

This rewarding role will see you support people on a short-term basis, helping and motivating them to make the changes they want to see in their lives. As well as interacting with the public, you will build relationships with Lewisham's local community groups and services and the wide range of professionals that refer into our project. This means you will get first-hand experience of Health and Social Care practice in London as well as an understanding of the sector and how it works. There will be opportunities to attend meetings and events, including forums with other Social Prescribers - helping you to gain an understanding of the rapidly growing way of delivering support.

We are looking for good listeners and positive thinkers, with exceptional abilities in organisation and administration. If you are passionate about helping those that need it, we want to hear from you.

Employee Benefits:

  • 26 days annual leave + bank holidays (pro rata for part-time)
  • Additional day leave for Birthday
  • Access to employee assistance programme, including access to helpline for partners and dependents
  • Flexi time scheme allowing the claiming back of additional hours worked (where appropriate to role)
  • Other flexible working options, including working from home (where appropriate)
  • Generous contractual sick pay scheme, allowing staff to recover without the worry of loss of income
  • CycleScheme members – enabling employees to save 25-39% of the cost of a new bike & accessories whilst also spreading the cost
  • TechScheme members – enabling employees to purchase tech through AUKLS and spread the cost from their salary

AUKLS is an equal opportunities employer and positively encourages applications from suitably qualified and eligible candidates regardless of sex, race, disability, age, sexual orientation, gender reassignment, religion or belief, marital status, or pregnancy and maternity.
