Job description

At Connection Support our team of staff members and volunteers work alongside people to overcome life’s challenges, with the goals of solving homelessness and achieving independence. We do this through our values of empowerment, collaboration, compassion and taking a personalised approach in all aspects of our work.

The mental health service at Connection Support is part of and funded by the Oxfordshire Mental Health Partnership (OMHP). Working in partnership with six local organisations we deliver mental health services for people in Oxfordshire. 

This role is based outside of the Connection Support setting in the NHS supporting out-patients attending appointments with professionals who are then identified as having some housing related or benefits need that we have the expertise to address.

The clients are patients of the AMHT who are receiving treatment for serious and enduring mental health conditions, mostly referred to the Embedded Senior Support Worker for short term interventions to prevent issues from escalating. 

We are looking for a Senior Support Worker who has a natural ability to connect with and support a wide range of people and can build positive professional relationships and networks with individuals, teams and partner organisations and work independently within the NHS setting. 

What you will be doing

You will provide patients referred to you with support to understand their situation and any issues that are impacting their life and recovery, developing a person centred and strengths-based support plan to find solutions and ensure positive outcomes for the individuals. 

Researching opportunities and providing information such as advice and advocacy on welfare rights, housing options, tenancy responsibilities, avoiding eviction and so on with the focus of developing their practical life skills and knowledge increasing their independence. 

Developing relationships and networking with other organisations is also a key aspect of this role, especially within the NHS, Oxfordshire Mental Health Partnership, other embedded Support Workers, plus voluntary and statutory agencies. 

To Succeed in this role, you will need

• A natural ability to connect with people, with excellent listening skills.  

• Experience of working empathetically, in a person-centred way with a diverse range of people and support needs. 

• Ability to research and seek information regarding local resources. 

• Find creative solutions for people. 

• Ability to mentor others and share information to develop individuals
