The Client Contact Coordinator are responsible for processing referrals and undertaking holistic triage assessments by phone. Where needed, referral on to appropriate services within agreed protocols and pathways.
Client Contact Coordinator are also responsible for inputting accurate core data in DHI’s client record data system, providing advice and signposting to professionals, partner agencies, families, carers and service users.
Provide a first point of contact for people referred or self-referring for drug and alcohol services.
Ensure a welcoming and motivating initial contact for all service users and carers, especially those who are new to the system.
