Our mission is to help children and young people have a fair chance to be who they want to be. We do this by providing a safe home, increasing life skills and self-confidence, and improving emotional wellbeing and mental health.
YMCA West Sussex Transitional housing provides safe, secure accommodation for young people aged 18-25, who have been, or are at risk of homelessness. Our staff team comprises of Supported Housing Support Workers, Mobile Night Workers and two Managers. We offer a support package alongside the accommodation to actively encourage residents into education, training, employment or voluntary work.
In delivering the role, you will work with the Deputy Supported Housing Manager and the wider housing team to provide effective day to day management of our transitional accommodation service. You will have your own caseload of residents, working creatively and effectively alongside young people to develop their assets and talents and to acquire the skills they need to live independent and fulfilling lives. You will contribute to the smooth and safe running of the service by providing consistency and reliability. You will have a calm, yet assertive manner, being able to handle potentially difficult situations. You will also support young people to safeguard themselves against abuse and risk. There will be times when lone working will be a requirement for this role.
This role is based on site in Horsham and will cover Transitional services across Horsham, Crawley, Burgess Hill and Worthing. Your working hours will be Monday to Friday, 9am-5pm.

You will already be a team player but also able to work on your own initiative, along with clear verbal and written communication skills, good IT skills.

Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. The Dementia Support Worker is responsible for managing a caseload of individual clients, providing practical and emotional support to the most isolated and vulnerable in our community. The support takes the form of weekly or bi-weekly visits to the client’s home, as well as contact by phone and email and liaising with social care and health professionals as directed by the client’s wishes or needs.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. We also work with several agencies and organisations in the local area to meet individual customers' needs. The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.
The Post holder will be expected to work on a rota basis (Monday - Sunday) and two sleep-ins per week which attracts £30.00 extra pay.The shift patterns are 8.00 am - 4.00pm, 2.00 pm - 10.00 pm and sleep ins.

South East London Mind is working in partnership with South London & Maudsley NHS Foundation Trust (SLaM) to deliver the Primary Care Mental Health Service for Lewisham. We have an exciting opportunity to work as part of an integrated team providing support to people with long-term mental health problems within a primary care setting, supporting people to develop their independence, self-management skills and achieve their recovery goals.
We are looking for a full time Peer Support Worker to work alongside SEL Mind and SLaM colleagues to deliver this service.
The successful candidate will provide recovery-orientated peer support to people with long-term mental health problems, facilitating access to community activities and emotional support.

An exciting opportunity for a support worker at one of our low need Learning Disability services in the Royal Borough of Kensington and Chelsea. Amy Garvey House consists of 11 self-contained flats providing accommodation to male and female customers. It is a low-need service, and the job role will involve providing support to customers to help them develop the life skills they require. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximize their skills and choices.

What you'll do:

- If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer.
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
- Participate in the support planning and risk management, as facilitated by the lead Support Worker
- Carry out security duties to ensure the safety of the customers and premises
- Enable customers to make full use of community facilities by providing support as directed
- Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities.
- Report any observations relating to customers welfare
- Adhere to all the policies and procedures of Look Ahead Care, Support & Housing plus those specific to the project including any statutory requirements
- Ensure Look Ahead Health and Safety policies and local protocols are adhered to at all times.
- Maintain records as required at the project under the direction of the Support Workers/ management.
- Participate in team meetings/reviews and the general development of the service

The Look Ahead's Hammersmith & Fulham Mental Health Service is an award winning service for 'Quality and Innovation'. We place a special emphasis on the Strength-based Recovery Model to enable our resident to achieve their full potential.
As a Support Workers, you will play a key role in supporting our residents to help them to enhance life skills and increase independence. A wide range of co-produced activities take place across the service, designed to empower and improve the daily living skills of customers.
Support Workers in the service work across all 4 projects in Hammersmith and Fulham; working with customers with varying levels of support needs. Many of the customers have complex needs including dual diagnosis and Support Workers are expected to liaise with other agencies within the borough in order to best support customers. Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available
Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation

Abbots Bank is a mental health, supported housing and community based service, working to a recovery model. We provide a range of support services for people with mental ill health to access in their local community, including outreach support, peer support and supported housing. We have been operating in the area for a number of years and offer a staffed service to our residents aged 18 and upwards.
We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in.
Responsible for helping and inspiring the people who use our services to maintain their tenancy and independence in the community, we’ll rely on you to work with them to come up with a personal support plan that will see them achieve their goals and aspirations. No relevant experience is required as full training will be provided, although candidates with an understanding of mental health issues and/or lived experience would be welcomed. The position is in Richmond at 21 Kingston Lane and consists of one high support building with 12 Customers.
The service provides a 24/7 support service, with an emphasis on choice and control, enabling customers to achieve greater independence, improved quality of life and integration into the wider community.
