Location: Hybrid role with Nominal office working in Chorley, and regular travel to venues around Lancashire for tenant visits, training, and team meetings.
Lancashire Mind’s Tenancy Management Service works in partnership with several registered housing partners across Lancashire to manage a portfolio of properties on their behalf and provide tenancy support to people living with moderate mental health conditions.
We care about making a difference to people’s lives and through the provision of regular support, we work with each person to improve their mental wellbeing and build the confidence and skills to sustain their tenancy and move on to independent living.
We use an outcome-based approach, making sure that each person is involved in their own support planning from the start of their journey with us and achieve this by offering encouragement and one to one support to help each person to understand and manage their tenancy, develop social and daily living skills, gain access to essential services to manage their mental and physical health and help and guidance to maximise their benefits and improve their economic wellbeing.
Your new role
We are looking for a new Tenancy Management Worker to join our small and friendly team to manage a caseload of customers who have complex needs including mental health conditions, substance misuse, homelessness, and a possible history of offending.
Your job will be to work closely with these individuals to promote independent living and help them to achieve and sustain their tenancies. This is a challenging and varied job which is often rewarding, particularly when the people we have supported achieve their own goals and move on to build better lives.
What you will need to succeed
You will need experience of working with people with complex needs ideally in a supported or social housing setting and have the resilience to support people with a diverse range of mental health conditions to overcome barriers to independent living.
It is important that the right candidate is an excellent communicator, able to listen and understand people’s needs but also confident communicating and liaising with other local services and partner agencies to ensure each person has the right support in place.
An excellent working knowledge of safeguarding practices for protecting vulnerable people is also key to this role and the successful candidate will need to be able to demonstrate proven experience of this.
Ideally, an understanding of the issues which arise within the social housing sector or experience working in a similar role would be an advantage.
You will need to be a good team player, with the ability to work well with others and be able to work on your own initiative with minimum supervision.
This is an essential car user role as it requires extensive travel to and from tenants’ properties in Lancashire.

About the roles:
These roles offer flexibility for home and office working with travel to appointments to meet clients across London is required. Office locations are Islington & Earl's Court.
As a Triage Caseworker you will cover shifts on a rota basis Monday to Friday 8am to 8pm and Saturday 9am to 5pm. You will make initial contact with clients explaining Victim Supports services & completing initial needs & risks assessment, and were required making ongoing referrals. You will need experience of delivering a telephone work and processing service with clients in a similar setting along with knowledge around safeguarding issues and legislation.

Job Summary and Role To fulfil the role of Youth Support Worker at Hungerford Youth and Community Centre. The main tasks of the role are as follows.
• To maintain the ethos of HYCC by providing a caring, safe and supporting environment for young people.
• To encourage and enable young people to develop and meet their potential as individuals and within their community.
• To organise and lead sessions for young people on term time Monday and Wednesday evenings 5:30 to 8pm.
• To attend staff meetings, usually on a term time Tuesday 1pm.
• To work in HYCC’s youth team to design a programme that ensures the Centre runs weekly session for: o Years 5 -6 o Years 7-8 o Years 9 and above o #ICE (club for children with additional needs). o Home ed. Club (for home educated children)
• To help run a programme of activities in some weeks of the school holidays.
• To organise and run occasional day trips for young people.
• To offer advice and support to young people, ensuring that young people are able to access up to date and appropriate information – including on emotional and physical health, sexual health, drugs and alcohol abuse, anti-social behaviour. This can include signposting and linking young people to targeted/specialist services, as well as written information and online support.
• To identify and report any safeguarding concerns relating to young people.
• To ensure that all work at HYCC is risk assessed and managed to minimise risk and maximise opportunity for young people, volunteers and staff.
• To ensure that young people who attend the sessions are involved in planning activities; to set up a young people’s user group/forum.
• To recruit and support adult volunteer helpers and young volunteer helpers.
• To keep and maintain up to date records of young people who use the centre – including contact and emergency details and consents.
• To ensure that sessions start and finish at set times, to enable debrief and feedback of volunteer helpers.
• To maintain any equipment used in sessions and report any losses or breakages to the Director of HYCC.
• To set in place and follow HYCC policies and procedures together with young people, trustees and volunteer helpers where appropriate.
• With Director of HYCC, to ensure regular user feedback – to report to and attend staff meetings and trustee meetings.
• Liaise and co-ordinate with other groups and agencies.
• To keep a timesheet of working hours.

Crisis Skylight Croydon offers IAG, 1-1 coaching and practical support to people who are experiencing homelessness in Croydon and the neighbouring boroughs. We have now received funding from NHS Southwest London to deliver a health advocacy service in Croydon for 12 months.

About the role
As Health Advocacy Worker , you will provide high-quality support to people experiencing homelessness in Croydon. You will support Skylight members as well as residents of local supported and temporary accommodation to register with the appropriate health services and attend health appointments. You will regularly visit local hostels and accompany your clients to their health appointments.
You will hold sensitive conversations about health needs, reasons why someone might have struggled to access health services in the past and what concerns they may currently have. You will provide IAG, support and encouragement to help build your clients’ confidence so that they continue accessing health services independently in the future when your support comes to an end.
About you
To be successful in this role you will have a good understanding of the challenges that people who experience homelessness might face when trying to access health services. You have experience supporting people with multiple needs including mental health, physical health and substance misuse difficulties, a history of multiple exclusion and social isolation. You have worked within a relevant sector which could include homelessness, asylum seeker or health services. You are confident working independently in the community and are willing and able to travel to meet your clients and potential clients around Croydon.

Role Overview:
Our Service at Stockport mainly provides support services to Looked After Children (LAC). The Service support the young people to develop independent living skills such as cooking, cleaning and personal hygiene.
