COMMUNITY CONNECTOR, MOVING FORWARD (CENTRAL NOTTS), NOTTINGHAMSHIRE, PERMANENT, 37 HPW, FRWK07 £20,133 - £21,866 FTE

Moving Forward is a county wide service that supports adults living in the community with a diagnosed mental health condition. The Service provides person-centred support that works alongside clinical and other statutory services.  We support people to address immediate issues around housing and money, and then work with them to develop the knowledge, skills and confidence to pursue broader goals (e.g. around healthy lifestyles, social connection, education, employment & digital inclusion) with the aim of achieving better mental and physical health and well-being, increasing independence and reducing reliance on services.

We are recruiting a Community Connector to assess and plan focused support to groups and individuals, with the aim of building and embedding knowledge, skills and resilience in people; to empower service users with the confidence and ability to access and connect with all community services, agencies and opportunities available to them. The role will also tackle and enable people to overcome barriers to digital inclusion. The post holder will be expected to implement creative and innovative support, including digital approaches, to help achieve success outcomes.

The role is based in Sutton-in-Ashfield but works across several county districts and access to a car and a current driving licence are essential.
