Moving Forward provides support to people struggling to cope with mental health conditions to regain control, build skills, knowledge and resilience to better manage their lives. Mental health is often present alongside physical health problems but both are less likely to be addressed. This post offers the opportunity to focus work on responding to where it can make a real lasting difference.
The successful candidate will be responsible for assessing and action planning service user’s health and wellbeing needs to complement their housing related support. The post requires a good working knowledge of services, agencies and activities related to health and wellbeing that would benefit the client group. Clinical treatment and specialist services are delivered by referring to external agencies.
The right person performing this role will be required to plan, organise and deliver their own work in co-ordination with the keyworkers to effectively achieve results for the service users. This may be 1-2-1 or group work around wellbeing and promoting healthy living options.
As the post holder you will need to be aware of the activities of other roles and services because their activity may complements or influenced your support. You will need to be flexible, creative and take a whole person approach to working with people with mental and physical health needs.
The individual will be responsible for a case load and will be expected to create, develop and deliver small group-work sessions around health and wellbeing to groups of service users. In addition you will be required to undertake other related tasks, such as organising Service User feedback meetings and organising or linking into existing external group activities for Service Users.
The right candidate should have a Level 3 Award in Education and Training (RQF) qualification or be willing to achieve within 12 months (This training is provided). A full driving licence and access to a car are required.
