A unique opportunity has arisen to be part of an exciting pilot to support High Intensity Service users in Nottingham City. This service will be delivered in partnership between Framework and Nottingham CityCare Partnership.
Due to the nature of this service, experience is required in working with people with complex needs as well as working within a fast-paced environment. You will work with a rolling cohort of patients who attend A&E or have been admitted to hospital more than might be expected, working collaboratively to reduce the number of inappropriate emergency attendances within Nottingham City.
We are looking for individual(s) whose ability to connect with citizens and will enable change in behaviours of those with even the most complex health and social needs. Skills required will be empathy, compassion, negotiation, patience and a real appetite for helping those that many others may have either forgotten about or have turned away.
You will also be involved with the continued development of this service alongside the Service Lead. This is an excellent opportunity to make a difference to people’s lives by putting in place interventions that reintegrate people into the local community without the need to make demands on services that may not be the most appropriate to meet their needs .
You will be working closely with the local Clinical Commissioning Group, A&E department, ambulance service, police, local authorities and local community health teams.
