An exciting opportunity to join the Nottingham City Drug and Alcohol treatment pathway/Assertive Transitions Service.
The post will: develop clear treatment pathways with local substance misuse services and mental health services
Identify and utilize appropriate substance misuse peer support and recovery communities
Provide substance misuse teaching and engage in clinical supervision with the wider assertive transitions service.
As an integrated part of the team, the SMCS will with a focus on discharge be part of ward round and clinical discussions, will conduct an initial assessment and review appropriate treatment options with the patient ready for discharge. The assessment will fully comply with NICE CG115 and DOH drug misuse and dependence guidelines and will include consideration of interactions between psychiatric medication, potential drug/alcohol misuse and proposed substance misuse pharmacological interventions.
The assessment and subsequent psychosocial treatment options will be conducted using motivational interviewing and will focus on building a therapeutic relationship, developing recovery capital and relapse prevention skills ready for discharge.
The resulting substance misuse recovery plan will be coproduced with the patient and include clear strategies to identify potential triggers, manage high risk scenarios and prevent relapse. The discharge plan will also include a clear plan in case of relapse in order to minimize its duration and effect.
This plan will form part of the wider plan of care.
You will be working directly with other Clinical Specialists, Drug and Alcohol Practitioners, mental health practitioners and be expected to assist across Nottingham City and Nottinghamshire.
You will have experience of working with people with drug and alcohol misuse issues and mental health.
Excellent communication skills and knowledge of/working towards knowledge of harm reduction models for Drugs and Alcohol are essential.
